Now I have symptoms....

Dear Dr.

I wrote to you about 10 days ago (see scared in china 12/6) At that time you informed me that my exposure was really quite safe and I was very grateful. About 24 hours after my post I developed some pain at the end of each urination. This was associated with some moderate frequency/urgency. There was no discharge or any lesion. This is not an unusual symptom for me if i pee right after masturbation or have vigorous sex. It went away in 48 hours. When I got home I received manual stimulation from my wife and i developed the symptoms again the next morning.

I got worried so I went to the local (USA) clinic and had my urine tested by 'PCR' for GC and Chlamydia. This was one week after 'exposure'. They also got an RPR and a UA. Everything was negative.

They advised me that I was safe but should see a urologist in the next month or two to find out what is going on.


My questions:

1) I was on cipro (500 bid x 3 days) in china for diarrhea. Could this have confounded my urine testing?

2) If the urine PCR is (-) at 7 days do I need to get restested? (I am having unprotected sex with my wife)

3) Have you ever heard of occassional pain after masturbation or sex in men like this? (I've had these symptoms before over the last 8 years)

4) Am I just being nuts??? My wife thinks I am overly anxious about this issue

The symptoms you describe do not sound like any STD.  To answer your questions:

1) Cipro would have cured gonorrhea and possibly chlamydia, and therefore could have made the tests negative.  2) Follow your own provider's advice about testing, but my best guess is it would be a waste of time and money.  3) Yes, this sort of story is common.  Almost every case in my experience was anxiety-related, not an STD or any other infection.  4) Maybe "nuts" is too strong a word, but your wife likely is right.

So...Bottom line. If I've tested negative and had the cipro before I came home does my wife need to get tested or can I continue to have unprotected sex with her and never think about this stuff again?

I cannot give specific advice.  I believe you do not have an STD and never had one.
