Unprotected (insertive) anal sex with transgendered escort/porn star

Hi,

Yesterday I met a gorgeous transgendered escort (no visible signs of drug use or health issues) and we had unprotected anal intercourse. I was the inserting partner, which I guess is an important factor. Prior to our engagement I had asked if she was drug and disease free and she replied "yes, I get tested". She is a porn star and also an escort and the industry has her test every few months (though I can't be 100% sure that she is still active in pornography, its more or less just taking her word for it.) She is a porn star though, I looked up videos of her but they were dated back from 2004-2007. She is an "upscale escort" and was bragging about having million dollar clients, so that put me at ease a little but I'm really freaking out here. I NEVER have unprotected sex and I don't know what got into me this time. I texted her back after because I was worried and she replied "OMG... I am clean, you will still remain the same as before, you are clean.."  I should probably trust her but I can't help remaining worried for some reason. I'm going to get an HIV and full STD test in 3 months but I really want to know what the statistics are or the likely hood that I need to be worried here. This is going to be a long 3 months!

UTI due to the rectal exposure certainly is a possibility.  Cipro plus doxycycline should be effective.

Since you have no evidence of any other STD, it is time to end this thread.  I won't have any further comments or advice.

Thanks. I actually went and saw a different Doctor because I didn't want to wait.

Blood pressure was 185/114 (probably caused by anxiety and stress)

Urine Check came back clear of immediate signs for STD's.

However, she checked again and under a mic I had white blood cells stacked in my urine, indicating I have a Kidney Infection.

She's put me on Cipro (I'm allergic to Penicillin and Ceclor.) and Doxycycline as a precaution.

She took my blood to test for any other possible illnesses, not sure what she would be looking for?

I know kidney infections are rare in men, so I'm assuming its A.) Because e. coli got into my urethra during the anal intercourse.  Or B.) which is just my guess until I see my doctor, something to do with the rectal bleeding / hemorrhoid.

These symptoms don't sound at all alarming. No STD. Sounds like anxiety, nothing more.  Mellow out. See your doc as scheduled; it's less than a week away.  Don't post anything more until then.

Thank you for the update, I really appreciate you following along with this. It's been about 10 days now and I've scheduled an appointment with my doctor. Unfortunately Its not until the 17th (earliest available) so I have to wait.

My symptoms have progressed actually. The bottom of the penis (assuming thats where the urethra is) is tender to touch, it feels though my Urethra is swollen. Likewise, I do have a burning / stinging sensation when I urinate and that sensation lingers for quite some time after urinating. It actually feels like urine is trapped in my Urethra but I don't think thats the case, I think its just the awareness of the walls touching each other from swelling. I have the urge to urinate frequently also but I'm drinking TONS of water so who knows.

4 Days ago I was stupid enough to get rather intoxicated for the Super Bowl. All day "drinking" and no food led me to have a 3 day persistent hangover. This is unusually long for me and on the third day I even had itchiness and small rashes (carried on through today, but most likely do to dry skin from hot showers I'm taking). Naturally, I freaked because headaches and rashes are symptoms of HIV. The headaches are gone however. During that 3 day ordeal I took advil every few hours and I've had hemmhorids that come and go and this time I had some moderate rectal bleeding do to one/thinning of the blood, though it could also be an indication of an STD, right?

Either way, those are my symptoms. I am definitely getting a persistent "tingle" and uncomfortable feeling in my urethra and sometimes pain at the tip, usually after I urinate... So I don't know what it is. Hopefully you can respond because I've got a few days before my doctors appointment to think about this.... Best,

Your analtyical tendencies, perhaps along with your "neurotic personality", seem to be working both in your interests and against them.  I agree with your analysis of the chance your partner has an STD.  At the same time, I doubt your urinary symptoms are due to any STD.  Chlamydia certainly isn't an issue, because it rarely infects the mouth and throat, therefore cannot be transmitted by performing oral sex.  And most of symptoms you describe don't occur with any urethral STD; they are more like genitally focused anxiety than anything else.  Or you could have a non-STD urinary tract infection, which can follow insertive anal sex. Urine color is determined by diet and fluid intake, not by any infection.

As we discussed above, it would be reasonable to see a health professional.  My guess is that nothing abnormal will be found, but better safe than sorry.  Feel  free to post a final follow-up comment if and when you have been professionally evaluated.  Otherwise that ends this thread.  Take care.

Thank you for the information. You've helped put my mind at ease somewhat.

It's been about a week now and I'm not sure what to think. Firstly, the person involved in this matter still tries to contact me with no show of concern, so I'm assuming the likely hood that she has something serious (and knows about it) is slim. Given her occupation, I'm going to also assume the likely hood she has something serious and doesn't know about it is slim. These are of course a lot of assumptions, however, It's this analytical thinking that seems to be keeping me sane for now hah.

So back to my current situation. I tend to have a "neurotic personality" so It wasn't beyond my thinking that I drank a lot of water so I could goto the bathroom just waiting for a stinging feeling to emerge, and carefully monitor it day-to-day. Of course, I immediately thought I felt a stinging feeling just hours after we did it. Then the next day it was normal. A few days later I woke up and there was a very slight stinging feeling when going to the bathroom in the morning but it subsided. Today it has become more frequent where I can feel it (or the need to urinate) more then usual, and its either symptoms or an STD, the side effects of me trying to go to the bathroom too much and possibly hurting myself, or the fact that I started taking new vitamins the day after this incident and my urine has been bright yellow since. I don't know.

The symptoms suggest the possibility for Chlamydia, if that's the case,  does it mean that the likely hood she could have had HIV also is higher? Since the odds of her having Chlamydia were only 1-5%? Or does this have no relevance to the matter.

I'm going to wait a few more days to see if symptoms become more frequent or painful because right now its nothing big and I'm getting tested in 2 months when I go home to MA where my medical insurance is. Other then the infrequent slight stinging and maybe the feeling of my urethra being 'full' like I need to urinate, there is no other signs physical or otherwise. Thank you.

Welcome to the STD forum.  With such a partner, you are right to be concerned about STD risks.  It is difficult for me to imagine how you decided to not use a condom -- but it sounds like you're having the same difficulty, so I won't lecture you.  Perhaps this will be a lesson learned, maybe the most important take-home message for the future!

Your story carries mixed messages about STD/HIV risk.  Members of the porn industry typically are quite aware of STD/HIV risks and are tested frequently. The same is true of many (most?) high-priced escorts.  From that standpoint your partner likely was truthful and probably is at low risk of having a current infection.  On the other hand, she allowed you unprotected anal penetration, so presumably she does that with other clients, and perhaps with her regular boyfriend(s).  That obviously is a high risk behavior.

With those conflicting considerations in mind, I cannot predict the likelihood your partner has HIV or another STD.  As a guess, maybe 1-5% chance she has chlamydia or gonorrhea, much less for syphilis and HIV (probably less than 1%, maybe as low as 1 in 1,000).  There's probably a 50% chance she is infected with HSV-2, but the chance she was infectious at the time of your exposure is much lower than that.  You can be pretty certain she has, or has had, HPV -- but so have you, since almost all sexually active people get it from time to time.  Remember that even if she is infected, STDs are not transmitted with great efficiency; that is, even if she has (say) anal gonorrhea, chlamydia, herpes, or syphilis, you were not necessarily infected.

Obviously, you need to be on the lookout for common STD symptoms, i.e. drip of mucus or pus from the penis, painful or uncomfortable urination,, and penile sores, blisters, etc.  If you have no discharge or painful urination within 2 weeks, and no sores within 3-4 weeks, you can be pretty sure you are home free -- but still should be tested.

You don't need to wait 3 months for meaningful test results.  A urine test for gonorrhea and chlamydia is reliable within 3-4 days of exposure, so you can do that right away.  And syphilis and HIV blood tests are fine at 6-8 weeks; a 3 month wait is old news for HIV testing.  You don't need testing for anything else.

Please visit a qualified health care provider for proper evaluation and testing, e.g. your local health department STD clinic if you aren't sure about your primary care provider's STD expertise.  Don't waste big bucks on some "we test for everything" panel advertised online.

In the meantime, don't freak out.  The odds really are in your favor.
