Red spots after oral contact

Hello Doctors,

I have a question regarding oral sex and STDs.
I had unprotected oral sex with an transsexual sex worker 19 days ago. I was very drunk that night, but I am sure that she only performed oral at me.
After I became aware of my risky behavior, 60 hours after the sexual contact I started to take doxycycline 2x100mg for 7 days. Now, 19 days after the contact (10 days after the doxy) I noticed two red spots with perhaps 3mm diameter at my penis just under my glans. They are reddish but the skin seems to be intact.
Do you think I have to worry about possible STDs, especially syphilis after what I described?
Would the 7 days of doxycycline be sufficient to prevent a potential infection (71 kg male)?
Are there any known doxycycline resistances?

I am looking forward reading your opinion on this.

Welcome back to the forum.

This and your previous questions indicate you are way over concerned about oral sex and STDs.  As discussed in the other threads, the risk of all STDs is very low from oral sex, and zero risk for some.

Accordingly, it was a mistake for you to take doxycycline.  You probably didn't prevent anything, and I even thin it is likely that your current red spots are indirectly due to that treatment.  Apparently you are uncircumcised, yes?  In that setting, broad spectrum antibiotics like doxy predispose to overgrowth of yeasts or other fungi, the main symptom of which,often is red spots of the sort you seem to have.  No STD is a likely cause, and syphilis never behaves this way (and in any case could not take hold in the presence of doxycycline).

So my advice is to stop the antibiotic, if you haven't yet done so; practice goog hygiene (retract the foreskin and wash with soap and water twice a day); then see a doctor or clinic if the spots persist after a week or so.

Hello Doctor Handsfield, thank you for your advice. I know that I am sometimes over reacting when it comes to health issues.
I stopped the doxy afetr 7 days, so I am without it 11 days now.

I visited a dermatologist this morning and showed him the two sports that now look reddisch inside and have a slighly darker border. The skin still seems to be intact (not wet or so) and the sports are still about 2-3mm in diameter.
The dermatologist looked at them with a magnifying glass and then took a swap for further testing and prescribed a cream containing 2% clotrimazole + 0,05% triamcinolone acetonide.
He said it is an infection of the skin and he cannot exclude an STD as the cause for this at the moment. I have to apply the cream 3x a day for 1 week now, then the spots should dissapear or I have to come back, he told me.

The medication is indeed for the treatment of a fungal infection I think, but I wonder if it is really a fungal infection why there is no itching. Is this possible in the beginning of such an infection that there is an absence of itching? I just hope that it is no serious STD like syphilis.

The dermatologist diagnosed a fungal infection, the only condition logically treatable with the cream you were given.  There is no STD that would possibly cause anything like this. So all things considered, this supports my initial impression above.  Syphilis is impossible for several reasons:  timing, nature of the lesions, and in the presence of doxycycline.

That will end this thread.  In the future, skip the doxycycline after sexual exposures unless professionally prescribed.  Best wishes.

Hello Dr. Handsfield,

I hope you don't mind when I write a new post regarding this topic. Things have changed a little bit science my last posting. Now the two red spots are lesions/sores of about 2mm diameter with a yellowish ground. The don't get bigger but now they are definitely sores (looks like "holes" in the skin with a yellowish ground).

When you rule out syphilis what could be a cause for those sores? Impetigo perhaps?

Herpes is the most common STD that causes ulcers (open sores).  I doubt that's what you have.  A superficial fungal infection remains most likely; modest progression for 2-3 days, despite treatment, isn't rare.  Beyond that, I really can't speculate on other (non-STD) causes.  I suggest you return to the dermatologist for reevaluation.
