STD\NGU?? Please help Dr Handsfield

Specific question for Dr Handsfield only please : I am 35 years old. I was stupid and drunk and away on business had met with a massage parlor and received the following. We had protected vaginal sex for like 5 minutes. She put on a condom but I don’t know if it was used or what but I can recall it was rolled on. She used her hand to put lube into her vagina. After 5 minutes She then took off the condom and gave me a hand job with her lube and possible secreations.  I finally finished masterbating myself.  I only have had 2 people in my life to have sex with one was 8 years ago(1 time) protected and then this exposure above. . The next day I had a mild burning\aching feeling deep inside my Urethra and tip of penis lasted thoughout the day. There is no change when I urinate but some times after urination it seems to burn a bit more but never painful also never any discharge. These are my symptoms and they have been like this for over 2 months, ALL started the day after my exposure. Here is my timeline in the next post :

I just got off the phone with the Lab tech at Kelsey-Seabold. The 2 STD Urinalysys they did was using a dip stick that checks for (Protein-Glucose-Ketones-Occult ZBlood-Bilirubin-Nitrite-WBC Esterase) She said if I was to have WBC then they do a Urinalysis with Microscope. Now I did this twice-  3 days after exposure and 23 days after both negative. They also did blood work for all the others negative.

- So since no microscope could it be an STD thats sneaky and the UA I did hasent seen it. I KNOW YOU HATE answering these questions so forgive me.

When doctors look under the microscope for bacteria they look for whitel blood cells at the same time.  I would be quite suprised if this was not the case during your visit.  Did you ask?

With the testing and treatment you've taken, even if you have persistent symptoms, you can be confident they are not due to an STD.  EWH

Last question - I seen the std clinic. The dr was surprised the "STD testing" my doctor did was only a urinalysys for WBC. There was no microscopic testing done. He did I full std panel for psa and urine culture for trich and microscope for any bacteria.

- Could I still have an std since the dr did not use a microscope?

** This is the last testing I am going to do and stick with it. Just wanted your thoughts on the possibility the wbc test was not enough and the microscope was needed.
btw 4 weeks ago I did take 1000mg Azithro and 7 days of Doxy twice a day. still have symptoms

I do not see the additional information.  As I said, your post needs to be briefer, not more detailed.  

Having said that, while I will not read your overly long description, my sense from skimming what you’ve written is that you have had urethral symptoms, primarily discomfort/pain for past two months starting the day after condom protected sex and receipt of masturbation.  Since then you have been tested on multiple occasions and, I suspect, treated without relief.  Skimming your note I see you have seen both a generalist and a urologist and have had multiple negative tests for gonorrhea, chlamydia and NGU.

This leads me to make a few general comments before I address your questions.  for starters, STD symptoms do not start within the first 24 hours following exposure.  This makes it less likely that your symptoms are due to an STD acquired during this exposure.  Secondly, while persons can have chlamydia, NGU or gonorrhea without symptoms including discharge, you have been tested ,multiple times making it unlikely that you have any of these problems.  Finally, ciprofloxacin, while a recommended treatment for prostatitis which your urologist has appropriately considered, is not recommended therapy for gonorrhea (the recommended treatment is an injection of ceftriaxone or oral cefixime) or for NGU. chlamydia for which the recommended therapy is either a single 1.0 gram dose of azithromycin or doxycycline, 100 mg twice daily for 7 days.  

With this as background, on to your questions:
1.  It sounds as though you have been tested for gonorrhea, chlamydia and NGU.  this being the case, if the samples were properly collected (using either a swab or a urine specimen collected at the beginning of urination at least an hour since your last void) there is little more testing to be done.  I am not sure that going to an STD Clinic or ID specialist would add much to this.
2.  It sounds as though STDs have really been ruled out.  Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turns leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  Perhaps this is a contributor to your situation.  
3.  If a swab specimen or a urine was tested for white blood cells, you were tested for NGU (see above).
4.  Your tests are negative.  It is safe to have unprotected sex.
5.  The Bactrim treatment was a therapeutic trial. there are no data to suggest that this will benefit.  From time to time we do have patients who have persistent urethritis. Following evaluation and treatment if symptoms persist, we have learned that we must simply wait these things out for them to go away but that in the interim, partners are not at risk.  As I said, this statement is true ONLY after carefully testing, which it sounds as though you've had.

Sorry, you do not get to choose.  Dr. Handsfield and I share the forum and for better or worse, you got me.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

3rd day after exposure:
Went to GP - he did a urinalysys for WBC count and blood work(STD panel). Urinalysis result came back that day and he said no WBC found so no Chlamydia\Gonorrea he said. After 5 days ALL the results came back negative from blood work for –HSV1 and HSV2 –Aids - syphillus. Acidity Chlamydia\Gonorrera

15thday after exposure:
Still having these symptoms throughout the day everyday some not so bad some a definite aching burning sensation some testicle dull pain. During masturbation felt a sharp stinging pain inside penis, ejaculation was no pain. I went back to my doctor and asked for another urinalysis and cbc. All came back negative.

17th day:
Went to urologist and he did a urine sample and said prostatitis. Urine came back “Clean” he said starting me on 2 weeks of Cipro. Not sure what test he did with my urine.

40th day
Finished Cipro treatment symptoms never changed. I went for a second opinion to another urologist. He put me on 1000mg single dose of Azithro and Doxy for 7 days (2 a day). He did a physical exam on my groin area and saw nothing. He ordered a ultrasound of my scrotum and he said no cancer or any abnormalties

60th Day after exposure
Followed up with Uroligist and he feels its mental pain caused by anxiety BUT I am a very busy man and I do NOT sit all day thinking on this so I cant accept after 2 months of this VERY noticible dull burning and aching in urethra and scrotum which all started the day after the exposure is just in my head or coincidental.

Questions:
1. Should I go to a STD clinic or Infectous specialist to do more testing like a culture or gram stain to identify the bacteria

2. What are your thoughts on what this could be. I feel my GP was not STD saavy and I read others who have Chlmydia no discharge and showed up in a urine swab culture and not on there intial WBC urinalsys.

3. Is this an NGU I feel they did not thoroughly test for NGU.

4. I started a relationship now and she is tested STD free. She wants to have sex without a condom but I feel its not safe until this is resolved? Is it safe for me to have unprotected sex with her?

5. I read 1 user with my same symptoms and length of time and same exposure finally use Bacterim orally and it worked for him.

6. Basically I am fed-up with this and want it to go back to normal. PLEASE let me know what the possible cause \treatment\testing I should have or course to take.
