Std Risk (Herpes and Hiv)

I visited a lady that provides erotic massage 14 days ago. After massage i regretted my decision but decided to move on and not worry about it. ( It included a good massage then she got on table in her panties while i was nude. she began to rub her boobs against my chest and then penis. while rubbing her boobs on my chest my genitals brushed up against her inner thighs and panties. She straddled me and gave an uncovered handjob to completion). Unfortunately, i began to itch in my hair area just above the penis 48 hours after the encounter. thinking i could have gotten crabs i applied permethrin 1%. Later that night began to feel some burning between my genitals and anus. symptoms have come and gone over the last 10 days. No blisters or lesions in 14 days. Just some itching and light burning. Could be anxiety. I am worried about having sex with my wife. What do you think is my risk for herpes or hiv from this encounter. I am usually pretty good about blocking out these funny feelings in my genital area but continue to get that itchy and burning feeling...Thanks for your help...

Thanks for the follow-up.  You have been to one of the country's best STD clinics.  All the physicians there are highly expert, as expected given Dr. Hook's expertise.  You can rely on anything and everything they have told you, and since they have directly examined you and I have not, their advice on your follow-up questions will be more valid than mine; same for your primary care physician.

In closing, almost certainly you did not acquire any infection at all from your massage lady.  As discussed in my original reply, your symptoms are entirely unrelated to that event, unless some of them are caused by your anxieties over it -- probably leading you to recognize body otherwise normal body sensations and probably natural variations in the appearance of your penile skin.  You should not expect further examination or testing by your personal physician to find anything abnormal caused by your erotic massage.  If s/he suggests counseling as the next step, you should follow that advice.

That will end this thread.  Do your best to put the massage out of your mind.  From a physical health standpoint, it was a non-event.

Last week (day 30 since my exposure) i visited our local std clinic and was examined by a very thorough doctor (dr hook is her boss by the way). She came to the same conclusion as you and even examined the red area that has formed on my penis. she did not seem very concerned and said the small pimple were some kind of sweet gland pimples. they automaticall test for hiv/syphilis which came back negative. i also got a herpes test from labcorp at day 30 which came back negative. not sure if that is signicant time to give a reliable result...i celebrated that day because the doctor at the clinic told me not worry and resume sex with my wife. my concern is that the next day i had a burning feeling in my urethra in conjunction with chills...another red scaly area has formed and i had chills this morning....i am visiting my primary physician in several days but there is something going on with my penis and these chills...the chills will last througout the morning and have gotten it twice in the past week.....what other bacteria could i have gotten from the encounter that would cause these reddish areas on my penis and chills?

You have misterpreted some things you think you know, or that you read on line, about herpes. Your symptoms do not suggest you have herpes.  I continue to believe nothing is abnormal with your penile skin.  If you are convinced otherwise, get examined in person at the STD clinic or by your own doctor.

In the meantime, do your best to accept the scientific evidence and my careful, reasoned reassurance.  Re-read my replies above, carefully and with concentration.

Feel free to report the result after you have been examined.  Until then, I will delete any further comments.

i am now getting scared....the tingling in my penis has continued and now i have a raised red area on my shaft in which i noticed several very very small pimples yesterday...they have not gotten any bigger today but another zit looking thing is forming today next to them....i have never had such on my penis....i have now freaked myself out that this supposed risk free erotic massage/hand job has now given me herpes....i had unprotected sex with my wife last friday and saturday....if i do have herpes....what are the chances i could have passed it on to my wife with this encounter....also, to ease my anxiety about hiv...i saw that a std center in my area will do a hiv dna test that can detect it in a shorter period..it has been 22 days since my exposure......what is ur recommendation....

These are typical symptoms of genitally focused anxiety, and if your doctor can't see anything on examination, you can be confident nothing is physically wrong.  You should resume sex with your wife.  You were not at risk, and I suspect returning to normal routine -- sexual and otherwise -- will help get you beyond this event.

thanks for your help doctor....it has been 19 days since my exposure to an erotic massage with panties on and a handjob...i know this is a zero risk type situation as u stated but i continue to have some itching in the genital area and some tingling on my penis....as i said before, i thought i could block out these sensations but they continue to come back...i guess my question is if i am on day 19 with no lesions....am i home free from herpes....these symptoms continue to worry me...i visited my primary physician and told him about my exposure...he was not very concerned and visually inspected my genitals...he even did a test for chlamydia/gonnorhea & urinary tract infection to rule those out(negative result)....occasional burning when i urinate....my wife continue to want to have sex but am scared to .....

Almost certainly she washes her hands between clients, but there would be no such risk even if she didn't.

Welcome to the forum.  You really needn't be at all worried about crabs or any STD in these circumstances.

First, the contact you had with your massage partner was much too superficial to expect any STD, and certainly herpes transmission wasn't possible.  As for crab lice, they cannot cause itching until the infestation has continued for at least 10-14 days.  The lice themselves do not itch; what happens is that an allergic reaction develops at the sites of attachment for feeding.  That reaction can't occur sooner than 10 days of exposure.  (It could be sooner if you have had crabs before.  But still 2 days is too fast.)  Also, if you had caught herpes, the symptoms would occur only at the site of inoculation of the virus, i.e. in a spot that had been in direct contact with a partner's genitals or mouth.  It's a fair bet that the area between your scrotum and anus was not contacted in a way that HSV could have been transmitted.

It seems most likely that your anxieties over the massage are making you notice normal body sensations that you would otherwise not notice or would ignore.  (Genital area itching of course is one of the most common body sensations experienced by all people.)  If the symptoms continue, it would make sense to see your primary care physician.  But I really don't think you have anything important, and I'm sure it's safe to continue unprotected sex with your wife.

I hope this helps. Best wishes--  HHH, MD
