Symptoms of urethritis persist - unsure what to do.


6 weeks ago I had an adult "massage”. In addition to a hand job - the lady licked my anus, fingered herself & fingered my anus & then completed the hand job. No other sex was performed.
I am male, married with a 1 year old daughter. I rang an STD clinic : said this was a low risk event - & I should continue life normally - i.e. continue sex with my wife which I did
1 week later I developed a sore throat & pustules at back of mouth.  Dr diagnosed this (did not inform of above event) as strep throat and prescribed 10 days of penicillin
4 days later: discomfort in my penis/ urethra, testicle pain and dysuria. Dr took blood and urine samples (came back negative) & prescribed azthromycin and another antibiotic for UTI.
3 Days later with symptoms still persisting I visited STD clinic. (Chlamydia, gonorrhea, & MG - negative) They said: very low risk event -  continue life normally, including sex with my wife.
6 weeks later: symptoms persist. 10-20 mins after urination: mild to moderate burning pain in my urethra. Dysuria persists. No penile discharge.
The Drs say: unlikely that I have contracted anything & I am imagining symptoms - they feel very real & I would have thought they would have subsided. I fear I have infected my wife, and worry about the implications for her health - untreated this could progress to PID. Should I tell her so that she can be tested (but rip the family apart ), or not  (given nothing has been found) to keep my family together and carry on with life - learning from this and being the best father and husband I can.

• Were STD tests affected by the antibiotics I was on and should I have them repeated?
• What is the confidence level for MG test?
• Any idea what (if anything) was contracted?
• Is this NGU, or prostratis - should I visit a urologist?
• Could I have passed something on to my wife & should I tell her  so she can be tested?
• Should I take more antibiotics?
• Any other advice?

Yes indeed, my answer was that your risk was zero.  Perhaps youi are not aware that her insertion of her finger into your anus is masturbation- it is.  EWH

I was looking specifically for clarification on the risks associated with the lady inserting her fingers into herself, and then into my anus (not the masturbation which I understand to be zeroo) Are you saying that this risk is also close to zero? Its that point I am not clear on.

We can quibble about whther the risks from masturbation is zero or so close to zero to effectively be zero.  Either way, there is not a medical reason to worry.

Yes, the antibitotics you received woud likely have cured any urethritis you had due to bacterial STDs.  Azithromycin is the recommended therapy for NGU, chlamydia and is effective for much gonorrhea, if you did have it.  Not sure what the other medication you had was but it would have only added to this.  EWH

Thanks Dr Hook for responding - I appreciate the advice - and your reassurance is certainly helping me move on.

I understand that the risks for masturbation are very low if not zero - but what about the fingering herself and then me?

I also understand that the antibiotics would have cured anything I may have had - but these were taken post sex with my wife - hence something could be passed back to myself.

Can you comment on these two points please.

Welcome to the Forum. It sounds like you got good advice from both of your visits to the STD clinic.   Receipt of masturbation and oral contact with your anus are not mechanisms for causing STDs. This includes that STDs would not be transmitted by transfer of an STD by a partner's hands.  Furthermore, I suspect the antibiotics you got would have cured any STD that might be present, including Mycoplasma genitalium  On the other hand, sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  This may be a contributor to your situation.  

In answer to your direct questions:

1.  Penicillin could conceivably have effected your tests but this is rather unlikely. both because penicillin has little effect on common STD pathogens and it is unlikely you were infected.  Even if you did have an STD, the combination of azithromycin and another STD would most likely have cured any STD you might have.

2.  Tests for M. genitalium are variable in their accuracy. Further however, azithromycin is the recommended therapy for M.g.

3.  See above. It is unlikely you acquired any STD from the encounter you described.

4.  I see no reason to seek further care.  If you do, you may get more antibiotics "just in case"  which is rarely a good idea.  It could be that you coincidentally acquired prostatitis but this is unlikely.

5.  No.

6.  I see no need for additional antibiotics
