Am I confusing this with something else?

Dear Doctors,
I have been with only 2 women in my life time. One I have been with for 4 years, and the other a close friend 5 days ago.  I have never had any problems so I am very nervous about this and want peace of mind.  5 days ago, I received oral sex, and she tried engage in sexual intercourse.  After about 20 seconds of realizing what I was doing, I pulled out and did not continue with oral sex or actual sex.  I am uncircumcised, and noticed a little pain when she was performing oral sex, maybe because she was pulling back on my foreskin too far. 2 days after this incident, the very tip of my foreskin was very sensitive in only one area. If something rubbed against it, clothing or boxers, I would feel discomfort.   Also, one pimple came up at the top of my thigh, near the pubic region.  I have bumps on my shaft of my penis, but they have hair coming out of them, they are white and very small. There are no bumps on my penis, and it has only burnt once when I urinated since the incident. Something that I have had in the past is penile papulla, around the base of the head, but very small and few in number.  It is still there and looks the same as it did a year ago.

I have not noticed any change in this area. I popped that one pimple, and it’s a bit sore, I have gotten pimples in this area in the past so I am not too worried about it. The things that concern me are the bumps where the hair follicles are, and why my foreskin is still a tad bit sensitive. It has improved in the past 2 days. I am having a lot of anxiety, because I have never experimented with anyone else sexually.   I talked to the girl that this happened with a few days ago, and she has told me that she is totally clean.  I have applied neosporin to the popped pimple and it has gotten better.  I have also applied neosporin to the foreskin.  Should I get tested for something?

I could use your description of "Loose stools sometimes diarrhea, and flu like symtpoms" to teach medical students about the physical symptoms of anxiety.

I don't mean to be glib about your question about discharge, but what source other than the penis enters your mind?  How many possibilities are there?  The anus if you had sexual activity involving that site, but that's more or less limited to gay men.  There's nothing left but the eyes, ears and nose.  I think not.

I never guarantee someone doesn't have an STD, but you describe nothing to make me suspicous of it.  You ought to chat with your provider about the obvious disconnect between the real risks and the level of anxiety you are experiencing.  Maybe counseling is in order.  I suggest it out of compassion, not criticism.

I won't have any further comments.  Best wishes.

final post:::
                 Also I have loose stools sometimes diarrhea, and flu like symtpoms.  should this worry me?  I am experiencing alot of anxiety as well, because this whole situation is scaring the hell out of me.  I also wanted to ask one specific question.  If discharge were to occur, would it only be from the penis?  thank you again for your help.

From your description, the foreskin "irritation" most likely is from minor trauma during sex.  But if the symptom persists, see a health care provider.  The white bumps sound like normal sebaceous glands or hair follicles.  Stop examining yourself so closely. If you have to look hard to see anything, then probably all is normal, and no STD presents with such subtle changes.

what do you think is the cause of the irritation to the foreskin?  It is still able to get slightly irritated?  Is that something to worry about?  Also, what about the white bumps that are small near the hairs on the shaft of the penis.

I forgot to say that pubic area pimples are very common and not likely to be due to an STD.

You describe a very low risk event with respect to STDs.  You seem to have an unrealistic view about the risks and frequency of STDs.  You describe nothing that makes me concerned about any STD and I see no reason for testing--and no reason for any worry, for that matter.

Of course see a health care provider of you develop STD symptoms (new genital sores, abnormal discharge from the penis). Otherwise, just let it go.  And the next time you're in a sexual situation with someone you know well and trust, just enjoy yourself.

Good luck--- HHH, MD
