Unprotected cunnilingus with sores in the mouth

Hi Doc,

I know, there are quite a lot of posts about the cunnilingus-issue, but my case differs a little bit.

About one week ago I met with a prostitute from an escort service in Eastern Europe. I had protected sex and unprotected cunnilingus with her. I didnt recognize any injuries or other uncommon things on her vagina, except some skin irritations in the bikini zone, probably caused by shaving. But I did not get in touch with them. The cunnilingus lasted for about 5 minutes and it was only before we had protected sex. The actions concetrated on the clitoris, so I didnt put my tongue into her vagina. I almost didnt get in touch with vaginal fluids - maybe only in small amounts, because she didnt get wet or didnt reached her climax. Her vagina seemed to be very clean, no smells or things like that. Also she wasnt on her menstrual period.

As I know from your previous answers and other websites the risk of a HIV infection caused by the described actions is zero or almost zero.

In my case the point is that that I brushed and flossed my teeth about one hour before the sexual contact. This caused small sores in my mouth.

Does this fact increase the risk of an HIV infection?????? If Yes, to which extend? The information I found on the web was not clear.

The problem is that I will meet my wife next week and it is not possible to get clear testing results before. Back home I got to have sex with her and there are no excuses for using condoms, because she is on the pill. We have two children and I dont want to destroy our life by telling her the truth if it is not absolutely necessary. On the other hand, I love her and dont want to infect her with HIV.

As you can see, I am in big trouble! Please give me some advice and thanks for your honest answer.

I hate to rain on your parade, but this is what the doctor said about STD with french kissing, I would suspect cunnilingus would be the same

It sounds like you might have had oral herpes, and if the fever was due to the same thing, it might have been your initial infection with HSV-1. However, you didn't catch herpes from the exposure you describe; herpes lesions cannot appear in less than 24 hours after exposure. Kissing can transmit syphilis, although the risk is much lower than with genital exposure. Syphilis symptoms take at least 2-3 weeks to show up. It also is true that the standard syphilis blood tests typically take up to 6 weeks to become positive--so your negative result at 4 weeks is somewhat reassuring but not definitive. If you know the man and/or can locate him, give him a call and ask whether he might have oral herpes or syphilis.

So to answer your questions:

1) The chance of catching syphilis from that exposure is low, but perhaps not zero, if your kisser happened to be infected.

2) French kissing is a good way to transmit or catch HSV-1, but your symptoms came on too soon.

3) The negative syphilis test indicates you probably didn't acquire syphilis during the kissing episode, but to be 100% sure you would need to be retested again at 6-8 weeks.

You might want to think about other STD's and not just HIV-maybe contact the agency and the lady and see if you can get info on her testing, if any

You are in no trouble. No one has ever gotten HIV from that and you didn't either. Forget it and enjoy your wife. Lesson learned. It isn't worth it, is it?

Your English is fine!

No case of HIV has ever been known to be transmitted by cunnilingus.  I am not aware of any research on cunnilingus in the presence of oral sores.  However, people have sores in or around their mouth from time to time (e.g., gingivitis, canker sores, etc), and there must have been lots of people who had cunnilingus with HIV-infected people but still didn't get infected.  Similarly, there must be millions of episodes of cunnilingus after tooth brushing and flossing.  Most likely your escort wasn't infected with HIV; but even if she was, you risk was extremely low.  Nobody can provide a guarantee, but I don't think you need take any special precautions with your wife.
