Unprotected sex one night stand

Hello I am a male whom had a one night stand with a female in which we engage in unprotected sex and unprotected oral, her performing on me. Also after sex she told me there was some break thru bleeding durring sex on her part.   This happened December 27th of 2013, after sobering up and having dialog it turns out she is a swinger and she says that she has a complete STD check every 2 months and is clean.  the next day i went to my Dr. and she gave me 1 gram of zithromax to take all at once and said its to soon to test for HIV or Herpes. I am married and want to put this behind me and resume my life with my wife.

Q1  Is the meds giving suffice for the bacterial STDs or should get tested,  or assume I am ok if I have no symptoms at the end of week 3

Q2 I have never been tested for herpes, should I be from this incident or does that muddy the waters.  If not how long should pass with no symptoms should i wait to consider myself home free to continue sexual relation with my wife.

Q3 I want to be HIV tested,  if i understand correctly i can take a rapid test at 21 days with somewhat accurate results, how accurate at this date?.  If negative at this point can i resume sexual relations with my wife?
will I need retested after this rapid test.

Q4 Can you keep this thread open for me to post my results

Yes, 4th gen is the same as duo or combo, and your negative results confirms you have neitehr antibody nor p24 antigen.  Four weeks is 100% reliable and I agree no further testing is necessary.

Ok I took an hiv 4 generation wRfx test @ 33 days which I assume is the same as a duo test you refered to and the results came back non-reactive under the result and non-reactive under the reference interval.  I assume that means I have no antibody or p24 antigen.  Assuming this is a duo test.  

I have had no lesions or discharge or any fever or illness since exposure.

My understanding this test after four weeks is roughly ~99.9% accurate and no future tests required and should move on.

I don't understand wha tyou mean about "actionable information".

A rapid blood test probably would be only around 70-80% reliable at 3 weeks; I think it is pointless for you to test that soon.  You correctly understand that the oral fluids test (which includes the home self test) may not be definitive until as long as 3 months after exposure.  It's probably at least 95% reliable by 6 weeks, however.  The oral fluids tests are the least suitable for checking for infection soon after an exposure; a blood test is always better.

If you really want quick reassurance, I would suggest you contact your swinger partner and ask her to have another HIV test.  If negative, you will no for sure you weren't exposed and don't need testing at all.  But if you rely on testing only yourself, there is no way you can get conclusive results earlier than 4 weeks.  Below are some threads that go into more detail about HIV testing performance at various times after exposure.

Thank you for your time

I was not given any test.  I was just given that medicine and that was it.

Do I need to wait 4 weeks for the rapid hiv blood test or can I get reliable results sooner from this. I am just looking for some peace of mind. If I can take the rapid blood test sooner then 4 weeks, how soon would be too soon?  It's been 5 days thus far.


If I understand you correctly the oral version of the rapid test takes up to three months versus the blood version of this test is actionable information now.

Are you comfortable with one using a home test that are sold over-the-counter?

I will post my results when they're available if I have questions.

Welcome to the forum.  Thanks for your question.

This probably was a low risk exposure.  Of course it would have been wise to use a condom for vaginal sex.  However, it sounds like your partner is unlikely to have any active STD or HIV.  As her own explanation implies, many swingers are very cognizant of STD/HIV risks and their prevention and people rarely lie about HIV/STD status and testing when asked directly.  So your partner's belief she is uninfected probably is correct.

Did your doctor test you for gonorrhea or chlamydia?  That would have been preferable to treating you with azithromycin.  To your specific questions:

1) Azithrmycin in a 1 gram single dose is very effective against chlamydia; pretty good against gonorrhea (~90% reliable); and uncertain re syphilis (syphilis resistance to AZM is very common in some geographic areas and nearly absent in others).  However, the chance of syphilis in this situation is zero for practical purposes, and gonorrhea almost always causes prominent symptoms, so between the treatment and lack of symptoms, you needn't worry about any of these STDs.

2) I would recommend against HSV testing.  The chance of infection from any single exposure is extremely low, especially in the absence of typical symptoms.  And the blood tests are not perfect -- just scan this and the herpes expert forum for lots of stories from people whose test results were more confusing than helpful.

3) Three weeks is too early to test for HIV.  If you are tested at all (which I really don't think is necessary except for reassurance), wait until 6 weeks for an antibody test, or 4 weeks for a duo test (for both HIV antibody and p24 antigen).  If you have a rapid test, make sure it's a blood test, not the oral fluids test.  That's the one that may take longest for a definitive result, up to 3 months.

4) All threads on this forum remain open indefinitely, assuming no excessive, anxiety-driven commments.

Bottom line:  If I were in your situation, I would not feel a need for any further testing at this point, for HIV or anything else; would not have a herpes test; and would continue unprotected sex with my wife without worry that I would infect her with any STD.
