Herpes 2

Dear Doctor,

My English is not good but I will try my best to explain to you, I am 21 years old, I made huge mistakes in London in 2012, I went to massage shop few times, and I had twice unprotected oral sex performed on me with the same sex worker, she was licking my scrotum as well, and kissing each other lips to lips, and also I had protected sex with a sex worker in a Sauna club in London, and these were my only exposures I had in my life and my first time to have sex. Then I became extremely worried about STD's.

After few days from the protected sex, I had a single red bump on my pubic hair area, above the penis, it was not close to the penis and it was not on the penis. Then I became extremely worried about Herpes, I showed that red bump to a nurse that day and she said it does not look like Herpes, but I'm not sure if she knows alot about STD's or not, then I had 5 HSV tests : First at 1 week, second at almost 3 months, third at 9 months, forth at 13 months, and the last one was at 13 months  as well. All the result were the same Positive for HSV1 and Negative for HSV2 and I know HSV1 is normal, I am just concerned about type 2.

The thing is I dont know why I cant believe my tests results, maybe because I feel guilty towards my girlfriend and do not want to harm her when we get married in the future ( I have not ever had sex with her ), someone inside me is telling me that the food you had or smoking or masturbation could affect the tests results.  

Anything happens to my body I start thinking about STD's and Herpes. Yesterday I noticed a swelling under the skin, it is on the top of the thigh, close to the testicle, the color is like towards brown, but its not that much, just slightly, I cant see it with my eyes , I just can feel it when I touch it with my hand, it is a bit hard. And I feel discomfort.

What do you think Doctor? Can this be Herpes?

This is me again, this will be my last post here, your comment really makes me better Dr Hunter, your last comment made me really better, but from yesterday I have been thinking if I explained to you very clearly about the single red bump that appeared after the protected sex or not, my English is not good so I dont know what should I name it, can I name it red bump or red pimple or a red boil or a red lesion or all of them are the same? because I dont know the medical name exactly of that bump or the pimple. The only thing I remember it was a single red bump or red pimple or red boil  ( I dont know what should I name it ) in the pubic hair area above the penis ( not on the penis) and it was not close to the penis.  

And just to let you know that I made an appointment with a mental health Doctor in my country as you advised me, but I wont be able to see him until I finish my exams and I go back home in June.

You are arguing.  You cannot win an argument against scientific fact.  You also are misunderstanding much of what you are finding by searching online; like many anxious persons, you are being drawn to information that inflames your fears and are unable to put that information into realistic context.  It does not matter how high or low your risk of catching herpes was; the blood tests and lack of symptoms that suggest herpes, taken together, are 100% proof you don't have HSV-2.  The tests done in London are the best anywhere.  Of course penis size can have no effect on any of this.  How would that be possible?

Your comments suggest an underlying emotional or mental health disorder.  It is not normal to be so resistant to scientific fact, common sense, and professional reassurance. Professional counseling would be a good idea; I recommend it from compassion, not criticism.

The thing is I read a lot about Herpes from the internet and in this forum, like some infected people would not get the symptoms and some of the infected people with HSV2 the test will not be able to detect their infection, and this HSV test is useless , these are the information that makes me worried, I feel that one day I will have the symptoms, caused maybe from the unprotected oral sex or the protected sex I had in 2012, I read also that condom does protect just 30% from HSV2, and the pubic area just get the infection by touching the partner pubic area while having sex.

I had these IgG HSV tests in London, are these tests as good as the tests in the USA? or the tests in the USA are better?

The size of my penis is small, would that increase the risk of getting HSV2 if she has it?

And any kind of food, coffee "caffeine", masturbation and smoking any substances definitely did not affect the tests I had? and would not affect the tests I will have in the future?

I had FULL STD's screen and HIV test at 9 months as well, all were NEGATIVE but I still really can not enjoy my day and can not focus on my studies and sometimes I feel I should break up with my girlfriend to not to harm her in the future when we have sex. And I always checking my penis.

Welcome to the forum.

There is no reason for you to be at all worried about herpes. First, you had a safe exposure that carried little if any risk.  HSV-2 is almost never transmitted by oral sex.  Second, if you had acquired genital herpes, you probably would have noticed typical symptoms.  Third, the symptoms you had are not typical of herpes, for several reasons; I agree with the doctor you sawy.  Finally, your blood tests show you did not catch HSV-2.  Your positive HSV-1 test almost certainly is from an oral infection in childhood and has nothing to do with the sexual exposure you are so anxious about.

Herpes never causes anything like the minor swelling you feel or a change in skin color.

I think you answered your own question about "why I can't believe my tests results " with "maybe because I feel guilty".  I'm sure that's the reason.  Whenever someone suspects his own symptoms or anxities have an origin like guilt or shame, usually he is correct.  And of course masturbation or smoking have no effect on blood test results.

All I can do is tell you what you already know intellectually:  you don't have herpes.  But you clearly do have an emotional adjustment problem.  If it continues, you should seek professional counseling about it.
