Urethra and bladder irritation

Hi Doc,
I am a 46 year old male who recieved unprotected oral sex from a CSW in Singapore in late Aug.  I became concered about STD's and had a stange feeling in my uretra and what seems to be my uterers and pressure in my kidneys.  5 to 6 days after exposure I was tested at a STD clinic for gonnorea, chlamidya etc and all came back negative.  The doctor also gave me 1 dose of azithromyacin, just in case. After the Azithro all symptoms dissapeared for about a week, and after that sypmtoms came back, a dull urethral pain, but no discharge. Pain was mainly near the tip and NOT during urination.  I followed up with my annual exam 3 weeks after exposure at my primary and my urine test came back negaive for blood and no growth after 48 hrs.   I followed up 5 wks after exposure at the STD clinic and tested for HIV, hepetitis, etc, all came back negaive.  The Dr said it would be a waste of money to do more testing as he felt I did not have any STD  Now almost 5 months after later, I continue to have a dull pain in my urethra and my bladder seems irritated.  I also feel pressure (no real pain) in my kidneys and what feels like my uterers.  After doing a bit more research, I have read that MG is cured 85% of the time with Azithroand 15% can go unresolved with this treatment.  
1) Is it possible that I could have contracted MG and the azithro didn't cure it?
2) Would a normal urine culture detect growth of MG or is this a seperate test?
3) Given my symptoms are there any specific tests that I should take?  
4) After 10 weeks I suddenly developed an intense headache ( I don't normally get headaches).  I went to an urgent care clinic and they did a CT scan and spianl tap with negative results.  I don't believe this could be related to STD's however I thought it was significant enough of an event to mention.  Is there a chance that this is STD ?

Thanks for your reply  What you explain is certianly reassuring and if I learn anything new I will post it back on this thread.  Thank you for the service on this site as there is a good deal of useful information in this forum.

Welcome to our Forum.  Mycoplasma genitalium is a recently recognized microbe which may be a cause of urethritis in some situations.  Just how common it is and its role as a pathogen is a subject of continuing study.  I can see from your question that you have research the possible causes of your discomfort and therefore I will go directly to your questions.

1.  Treatment response to azithromycin is, indeed, less than 90% and thus, if MG were present there is a chance that you would not have responded to care.  However, if MG were present and causing your symptoms it would be most unusual for there not to be SOME sign of inflammation in your multiple urine tests. This would present as a significant number of white blood cells detected in the urine.  That this was not seen makes MG far less likely.
2.  See above.  Tests for MG are not widely available and currently are largely research tests.  On the other hand, it would be expected to see and abnormality.
3.  No, see comment below.
4.  My goodness, that was an aggressive work up. Glad it was negative. These problems do not bring to mind any specific STD.

A final comment.  You may have had NGU.  If so, sometimes complete resolution takes a while but you are now out beyond where symptoms would normally be seen.  you have been tested on several occasions with negative results and treated with no lasting relief.  Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turns leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  Perhaps this is a contributor to your situation.  Just something to consider.  Most importantly, with the test results you have, I suspect this is not something serious and is not an STD which puts you or your sex partners at risk for further problems or complications.  EWH
