Very Worried

I received unprotected oral from stranger.12th day HIV DNA PCR -ve.Avoided sex with my wife for a month.28th day HIV 4th gen -ve.Then one day after masturbation,had a very weak ejaculation,pain in the testicles,burning urination.No discharge.2 days later developed a sore throat.All these symtomps reduced in 5 days.Mild testicle discomfort on the right side and mild sore throat continued.Resumed protected sex with wife.Never had a condom tear.After the third intercourse,my partner developed uti symptoms.She earlier had a fungal infection, but now it was more severe-abdominal pain,dark turbid urine,burning/frequent urniation.Urine routine showed pus cells.She was put on nitrofurantoin and Metronidazole.A week later she started feeling fine.After our next intercourse she again developed irritation while urination, etc.Symptoms were mild.Urine culture came out clean. In panic,I had Azith 1g+cefixime 400 as a single dose.My partner had too.After a couple of weeks, after intercourse, the irritation while urination returned!This time the culture showed positive for proteus mirabilis with a resistance to nitrofurantoin.She was treated for this.These results were very confusing as the previous culture was fine. Her pelvic+kidney ultrasound was normal.At three months,I took HepB (core-Anitb+Antig),VDRL,HIV-ELISA, all -ve.Wife's irritation while urinating after our intercourse lasts a day or two,and has now become a regular feature.It has now been 6 months.Gynacs say it could be latex allergy.Also,my testicle discomfort and sore throat continues.The discomfort is on and off.Visited a urologist,and he ordered a scortum ultrasound which showed no problem.Asked me to get back if things get worse.My 6month result for GONORRHOEAE-DNA-PCR,herpes1/2(blood) IGG are -ve.I am very confused and unsure whether my testile discomfort and sore throat are any std.I cannot make out the reason for my partner feeling the way she is.If I was your patient,what would you recommend me as next steps.

Sorry, it was late when I answered your question.  This should have read "to stop worrying..."

Regarding your continuuing questions:
1.  These symptoms wer unlikely to be STD.  The most likely cause of such symptoms would be UTI but of course there are other non-STD possiblities. Only she and her doctor can determine this.

2.  Yes, you can stop using condoms at this time.

3.  No, HPV is typically asymptomatic and the symtoms you describe in no way suggest HPV.

4.  Sorry, you asked and got my assessment.  Guilt is a powerful force.  EWH

Thanks doctor. Just a clarification, the "not stop" is a typo or do you mean I have to worry about the infection.

I have a follow up questions.

1. Her recurrent uti-like symptom could be something else, and std can be ruled out?
2. We are planning on a baby now, is it safe for me to stop using condoms now.
3. HPV is the only major untested STD. Is it possible this is causing all these problems. Besides, is hpv transmitted inspite of condoms.
4. Its very hard to believe all this is unrelated. The testicle discomfort is on for 6 months. We both were very healthy prior to this.

Welcome to the Forum.  You are considering a variety of unrelated events and allowing them to worry you that you might have and STD which you transmitted to your wife. That is not the case.  HIV is not transmitted by receipt of oral sex, few STDs are transmitted by oral sex, and your STD tests prove that you do not have an STD.  Further, the therapy you took would have cured the most common STDs, were they present.

Your symptoms are not suggestive of STD when considered in the context of your tests and treatment.

As for your wife.  UTIs are related to sexual contact- whether or not condoms are used.  The action of intercourse can lead to introduction of bacteria into the female urinary tract.

In addition, her physician's suggestion that she might have a latex allergy is quite reasonable.  About 9% of persons regularly exposed to latex develop allergies and her symptoms are consistent.

My advice is not stop worrying that your infidelity might have led to infection or caused infection in your wife. You have clear evidence that this is not the case.  EWH
