very worried

Dear Doctors You know about my case with the unprotected vaginal sex with that girl.What I forgot to tell you is the fact that I also performed Cunnilingus on that girl.I know that cunnilingus is zero risk for HIV but not for other STDs.
My symptoms are now sore throat and  joint pain at my right elbow and left knee and muscle pain all over my body.No fever.No rash. No dysuria.No urethral discharge.No pain or swelling down there . You told me that I should get tested for other STDs so I will.I will do at 4 weeks my hiv test(4th generation).Now I have a lot of questions for you.Just be patient with me.Here they come :
1. A syphilis test at 4 weeks after exposure will be ok to do or is it too early?
2. What STD contracted by cunnilingus on a woman can cause me this sore throat ? Can I Have Gonococcal pharyngitis? Or yeast? Or Chlamydial Pharyngitis?Or..what?Should I do a throat swab ?
3. Regarding my joint pain at my elbow and knee...could this be Reactive Arthritis (Reiter's Syndrome ) ?
4. I am planning to get tested for Chlamydia and Gonorrhea .I am very shy and have a very sensitive penis and I heard that an urethral swab can be very painful and there is a discomfort from my point of view doing this.The question is:Can I do an urine testing for both of them so I can avoid the urethral swab or the urethral swab is absolutely necesary?The lab in my city(In Romania) does DNA-SDA Chalmydia and Gonorrhea detection?Is this test ok ?
5. What about ureaplasma urealtycum?
6.What is my risk for mono ?I had no fever.Only sore throat.Should I get tested for mono?

Gonorrhea and chlamydia testing are accurate within 3-4 days after exposure.  You can be tested tomorrow.

I forgot to ask you one more question: When should I perform the tests for Chlamydia and Gonorrhea? My exposure was a week and three days ago.
Should I go tomorrow and do the tests or should I wait one more week?

Welcome to this forum.

STDs are very rarely transmitted by oral sex on women (cunnilingus).  That part of your sexual exposure does not change my opinion or advice.

1) Syphilis is very unlikely, if you don't develop a sore (chancre) on your mouth or penis.  However, 4 weeks is a bit early for a syphilis blood test.  6 weeks is better.

2) In theory, gonorrhea is posssible.  However, it is rare from cunnilingus, and gonorrhea usually does not cause sore throat.  Most likely your sore throat is just a common cold, unrelated to the sexual exposure -- or maybe a cold virus from kissing your partner.  You could talk to your doctor about doing a throat swab to check for gonorrhea, but you can expect it to be negative.

3) Your knee pain has nothing to do with this sexual exposure.  It is much too soon for reactive arthritis.

4) Testing for urethral gonorrhea and chlamydia is usually done on urine and the SDA test is highly reliable.  A urethral swab usually isn't necessary.

5) Ureaplasma is a normal organism in the urinary tract and usually causes no health problem at all. Don't worry about it.

6) The risk of mononucleosis from any single sexual exposure is extremely low, probably under one chance in a million.  Don't worry about it.

Just remember that the large majority of sexual events with commercial workers do not result in any STD at all.  Getting tested makes sense for reassurance.  But really, you should not be worried about the outcome.  Most likely all tests will be negative.  Feel free to return to let me know the results, but I won't have any other comments or advice until then.
