Every woman has to get a pap smear done for her own health to make sure everything is fine. A pap smear may be uncomfortable, but it should not be painful. You can talk to your gynecologist about being nervous and they can talk to you about everything they will be doing and can use a smaller instrument to try to make it less uncomfortable. And as said above, you may or may not get some cramping or spotting, but that is normal and nothing to worry about. Just try to relax and talk to your doctor about your worries.
What is it exactly that has you so worried? I understand the entire procedure is awkward and uncomfortable, I've been there. But the biggest thing is to relax. Your doctor has probably done a million of these. It's not painful if that is your worry. As long as you keep your muscles "down there" relaxed you'll be fine that's the only cause of discomfort I ever had. After it's done you may feel a little crampy and possibly some very minor spotting. Really nothing to worry about.
