yes go to look on meds prescriptions.com and type in the pill you would like to look up
for medical records offices are only required to keep them for 7 years... you may not be able to get that info for that long ago and i dont know of a way to get any of it if you dont remember the doctors names. or at least the practice.

Officially records are usually kept only for 7 years after the last visit of the patient. If it was 25 years back and you have not got the prescriptions yourself, do not remember the doctors’ names, then the only way to update records is ask the people around you if they remember—you suffering from any illness, medicines you took, doctor you went to etc. Obviously you have some serious medical issues where past history is important. Also try and recollect yourself and make a date-wise record. Hope these tips help. Take care!
