Looking for some reassurance, or to prepare for the result.

Hi all I’m a male, 41 years old that is looking for some reassurance.
I'm waiting for test results taken today (urine sample) but for the last 3 days I have had blood in my urine. No pain when passing urine apart from a little discomfort in my lower abdomen. But each time I go there's a lot of white filament/floaters (some red (blood clots?)) in it.
These floaters I have noticed in my urine as dull white/grey for several months & there would only be a small amount during those months at each time. Nothing compared to what I'm getting now, obviously a lot of it may be puss. My GP thinks it could be a urinary infection and has put me on antibiotics and I'm to see him in another week to discuss the results and also if the bleeding hasn't stopped he'll carry out further testing including prostate tests.
I'm noticing today that within a few minutes of passing urine I'm getting some seepage which stains my underwear a yellow (puss?) mixed with light red splotches that must be blood. Sometimes I have this seepage coming a few seconds after the initial urination stream and I’ve noticed that it seems a thicker that the urine I had passed.  I’ve had no burning sensation when passing urine, I have no back pain although the other day I may have had a fever overnight. I do get fairly tired during the afternoons (sapped of energy at times) but have always put that down to my age (41 years). I’ve suffered from depression for most of my life so often put my fatigue down to my mental state.
Two days prior to the blood in the urine, my urine seemed normal (well what I was used to) but my ejaculate (Sperm) had blood initially. 1st experience was a brown colour mixed with my seamen then the next day it was bright red almost seemed full of blood and then that same day I noticed the blood in my urine.
Over the preceding months I did notice that my urine smelt overly strong, almost as if it was more acidy or I’ve noticed on some forums people have stated theirs smelt like ammonia which could also describe the smell. But thought nothing of it, until now of course.
So I guess I’d just like to put my mind at rest and find out if this does sound like a typical urinary infection and if so does it sound like it might have gone to my kidneys as I’ve heard that can be fairly serious if it has. As Prostate cancer is a scary thing and I used to know someone in his late 60’s early 70’s that confided in me once that he found blood in his urine and knew that it was going to be cancer. Within the next month not even two months he passed away with prostate cancer.
Which is why I’m a little concerned atm. Not much I can do if it is, but I guess a part of me is hoping that if it isn’t then all well and good. But if it is more likely to be prostate cancer then I’m hoping for a heads up so that I can put my affairs in order and finish/do what’s most important for me in life while I’m still here.
Btw, I have been married for 6 years and my loving wife is the only woman I have made love to during that time.
Historically I have had a urinary infection about 6 or so months ago where I was peeing razor blades but there was no blood. But it was about the time when I 1st noticed the stringy white filaments in the urine. At the time they were several centimetres long but not as much as what I’m now getting every time I go. I had a urine test to check for gonnorea, clymidia etc. Blood tests done as well.
All I got out of that was that I did have some kind of infection and all the tests came back negative and seemed normal apart from some slight indications of liver damage but she didn’t go into any detail over the phone and I didn’t press her as I took the call from work. She asked if I drink a lot and I said only occasionally about 2 cans of scotch and coke during a week and not every week at that. She just suggested that I change my type of alcohol.
Now when I was 9 (32 years ago) years old I was involved in a bad car accident in which I suffered a torn bladder amongst other things. I also at the time of being hospitalized had a catheter inserted into my penis which they had left in their too long. I think with hind sight that maybe some scar tissue had started to grow around the inflated balloon of the catheter and when they withdrew it they tore away some tissue with it. Let’s just say I was peeing razor sharp razor blades for months sometimes barely able to hold myself up whenever I went.
In any event, I’ve mentioned these episodes as perhaps they could be related to what I’m experiencing now.
Sorry for the long winded question, I try talking to my wife about it but she’s dealing with enough of work and everyday stress and her knowing about the blood in my urine is on her mind as well.
So I’m asking....should I be worried that it’s indicating something more serious? Should I prepare myself for the worst?
Many thanks for your time.

Thanks for your post Jen. I'm in Australia so not sure about whether our health system can cover that or if they can even test it. I'll keep you posted in a week or so of any developments, I have one more week of anti biotics and then I'm off to see my GP to find out what caused it all. The bleeding has stopped well and truly, still a bit (although) smaller filaments floating in my urine and my stomach still feels funny and at times a little painful when I ejaculate (stomach hurts as the sperm comes out). So a part of me thinks it could be my prostate that's causing the pain but I'm hoping the GP can carry out the relevant tests to prove it one way or the other. Because I definitely want to know where the bleeding came from, where as you've suggested where the filaments are coming from (which could be normal I guess). Again thank you for your time and keep well.

No my GP hasn't suggested anything like "interstital cystitis or blockage from scarring " other than mentioning it sounded like a urinary infection when I saw him on Wednesday. I was his 2nd last appointment for the day and personally I think he just wanted me out of there. But to be fair he did follow up quickly when he received the results and tried to contact me earlier on Friday (yesterday here in AUS) to tell me I needed to change my medication. He did mention in my quick 5 minute session when I saw him that I may need to see a urologist but it would depend on if the blood in the urine or other symptoms would clear up after my course of meds which at that time was the Doxycycline. Now I'm on Amoxycillin. Apologies for repeating info from my previous post. Thanks again for your time and now I'll have to google interstital cystitis :)
Take care Jen.

PS: Apologies to all for my spelling mistakes from all my previous posts.

Hi all. Thanks very much for your responses. I realy appreciate your time and effort as I found them informative helping me come to terms with what it could be.

A bit of an update, yesterday my GP asked me to call him (message via my wife) which I did in the afternoon on the way to a job interview which I ended up being late for and had to reschedule for Monday but oh well things happen.

So I called him and he told me that he'd received the results of my urine sample which I had only dropped off first thing the previous morning. So I'm impressed by the speed of the results which I'm thankful for. He informed me in the short phone conversation that I did have a urinery infection according to the results, he didn't explain how bad but he wanted me to immediately start a course of anti biotics other than the ones he had initially prescribed which I had just started taking.

He explained that the 1st set of Doxycycline 100mg he'd originally prescribed were a general anti biotic that covered a small range of conditions. He now wanted me to take Amoxycillin Sandoz 1000mg tablets over a course of 14 days after which he wante to  me to see him. I asked him that now that he knew it was a urinary infection whether it was something that I should worry about all he said was something similar to "well, we'll wait and see."

Which I guess is fair enough as no one could probably tell what type of infection, the cause or the location of the infection without carrying out a series of tests. I'm prosuming that he wants to clear the symptoms first with the anti biotics and then discuss what's to happen next.

During the call he didn't stipulate what bacteria if any had been found but I intend on askingn him if he could refer me to a urologists as having two urinary infections within 6 months (not even) can be a good thing unless I never really got over the 1st one.
In any case thanks again for your time and I'll post an update later when I know more for those of the community that may be interested or going through something similar.

At this stage, this morning my urine has returned to a normal light golden yellow colour (from the dark orange brown it had been) but I'm still finding plenty of white stringy filaments although now none are red. However this morning my sperm was still mixed with blood and after ejaculation I had a stronger feeling of discomfort bordering on a slight bit of pain after ejaculating. (Apologies for being too descriptive.)

On that note. Thank you all again and please take care. Live life to the full and appreciate all those involved in your life no matter how small.

Best Regards.

MEDICAL PROFESSIONAL
Hi
Welcome to the MedHelp forum!
The sludge or substance you see in urine could be bladder debris. Parasitic infections like schistosomiasis and filariasis can result in worms in urine which appear as fleshy substances. Infections like trichomoniasis can produce mucus threads in urine. Fungal infections like candidiasis too cause fleshy substance in urine. Bladder cancer, polyps and stones are other reasons. You could in addition have a urinary infection of both urethra and bladder and this should be investigated as soon as possible. Hence wait for the test results. Maybe if there is pus, a culture should be set up to identify the organism and a sensitivity done to identify the antibiotic that will act best. Blood in semen and urine is usually due to non specific infections but the possibility of cancer cannot be entirely ruled out. A non invasive test such as CT scan should be done first.
Do discuss this with your doctor and get yourself examined.
Hope this helps. Please let me know if there is any thing else and do keep me posted. Take care!
