The symptoms could be exacerbation of the GERD. It could also be hiatal hernia. Treatment is a combination of drugs to reduce the acid and lifestyle changes. You will need to take a combination of medications (under medical supervision) like a proton pump inhibitor such as omeprazole, empty stomach in the morning and an antacid gel after meals for complete relief. Possibility of H pylori infection too should be looked into by a carbon urea breath test and a combination antibiotic tried. Many a times a persisting H pylori infection can be the cause behind acidity not responding to treatment.
Life style changes that will help include: Avoid heavy meals and eat frequent small meals. Avoid too much of caffeine, tea, smoking, fried food and drinks both alcoholic and non alcoholic fuzzy ones. Avoid heavy exercises within 4 hours of a heavy meal. Raise the head end of the bed by pillows to 30 degrees. Avoid lying down for least 2 hours after food. Maybe these tips will help you.
You should consult a gastroenterologist (a specialist who looks after the diseases of our digestive system) or a physician for this if these tips reduce your symptoms. You may need upper GI endoscopy for confirmation and to know the degree of damage.
Liver function, pancreatic function and electrolyte imbalance too should be investigated if the above suggestions do not help.

I'd suggest returning to your doctor and making sure you delineate all your symptoms and get thorough testing.
Swallowing and hearing "crackling" in your ear is likely due to fluid. (I'm in the same boat). Fluid in your ears can be from a load of different things, but in your case it would be chronic and is most likely from allergies. Most of your symptoms could be related to allergies actually - unrelenting cold symptoms, nausea from mucus draining into the stomach (it's totally normal, but if you have excess mucus draining it can give you an upset tummy), blurred vision.... Back pain, not so much, unless you're holding yourself stiff because you just feel lousy.
Poor oxygenation related to tenacious mucus could also be increasing your anxiety. Until you go back to your doctor, try increasing your water intake to help thin mucus. When you _do_ go back to your doctor present your symptoms as concerns rather than complaints - it might get you farther:
For example, "I'm a little concerned about my inability to maintain a normal diet. The nausea from eating is interfering with all of my intake. Could this be related to the fluid / crackling in my ear? " (etc).
you should see a pshyciaterist 1st and then he will guide you what to do....because you hae anxiety so may be these problems are due to your inner tensions....
Ok- well a sounds like anxiety that leads to panic attacks. First are you taking anything for that? If you are that could explain your eating issues... on the flip side... if you are not that could also be your stomach issues.
The crackling noise in your ear when you swallow- do you get headaches? If so how often? Do yo clench your jaw? How long has the crackling thing happened?
Umm... I would suggest you stop staring...that would happen to anyone if they stare too long.
As for the long lasting cold... could be that your anxiety level decreased your immune system... hence the cold wont go away. Or you may have an issue with your immune system. Ever had mono? Been tested for Epstein Barr? Ask your doc to have that tested if not. In the mean time... first try yoga (at home is fine) or meditation or breathing exercises... or hell a bath & glass of wine... anything to CALM DOWN when you feel worried.... that alone may help your cold and your mind...
Trust me I know anxiety and panic attacks are a major PIA... but if you want to feelbetter YOU must make up your mind to learn to help control it so it doesnt control you.
