Hi
Welcome to the MedHelp forum!
The swollen gland could be throbbing because of infection or it may be an infected cyst or a gland. It is really difficult to comment without examining. If at all there is a narrowing of the carotid or there is a clot there then it is possible that running etc causes dilatation of the lower end of the artery.
Please consult your PCP and get a referral for a head and neck surgeon or for an ENT specialist. It is difficult to comment whether the symptoms are due to ear infection or not as the  nature of swelling cannot be ascertained on the net.
Hope this helps. Do let me know if there is any thing else and keep me posted. Take care!
