thank you - my son is 37 so I guess it is pretty impossible to get his immunization record.
That does make things difficult- was there a second doctor working with him? I think by law they have to keep records for a few years before destroying them so even if the dr died, they wouldn't be allowed to just shred all the documents. Sorry I'm not more helpful.
Thank you. Unfortunately my son had a private doctor and he is no longer either alive or working.
You have to go straight to the source. I had to go to my former dr's office and sign a release form to say that I wanted my medical records released into my care, or a different dr's care. Even if his former doctor no longer works there, the records should still be at the clinic. Most practices keep records for at least 10 years, but some might be longer.
