A bruise on penis or penile bruise usually occurs due to rough handling of penis either during sex or during masturbation. It could also be due to deep thrusting during intercourse. If there is loss of erection, then it could also be penile fracture. Clots in veins and arteries could also be a cause and so also infection of veins and lymphatics.
It could also be allergy to condom or to any other thing that comes in contact, that could be causing urticaria and bruised look. It is difficult to diagnose this on net. Please consult a doctor.

Depending on how bad it is you may want to have him go see a doctor. If he is able to function and there is no damage to his shaft then you should just let him heal. Its probably a good idea to lay off the sex during this time.
