This is something which should be taken up with a doctor ASAP or go to the ER. Either your father has exercise intolerance due to diabetes or hyperthyroidism, or there is blockade in the arteries of the heart causing angina or there is severe mitral valve prolapsed or there are other heart related problems. A comprehensive heart examination including ECG, ECHO, tread mill test and probably angiography will be required. Please do not delay going to doctor.

As doctornee stated, he needs to get a physican examination as of yesterday.
The problem could be simple dehrdration or a low hematocrit, but the comment "when he starts to sweat" concerns me.
Cold beads of sweat suddenly appearing on the forehead, juxtaposed with extreme weakness (or fainting) are hallmarks of cardiac ischemia. In other words the heart isn't getting enough oxygen. Why does this cause beads of sweat to appear on the forehead? Nobody is quite sure. But it is one of those things that should cause you to ring up "general quarters".
That leads me to the conclusion drnee has reached. A comprehensive heart examination is indicated.
