help

I have an 18yo friend who was in a horrific MVA 09/2004, crushing her backbone and pelvis. She required a cervical cone d/t bleeding at that time. On 12/09 she had a positive pregnancy test. That is the beginning of her hell. On 12/27, 2007 she noticed, bright red spotting. Spent that night in Er, where diagnosed with inevitable miscarriage, despite the fact they told her she was not dialated and they were unable to visualize her uterus or left ovary via transvaginal us. Rested that whole next am and Saturday decided she had to go to work. After getting ready for work that afternoon, she started bleeding dark red blood with clots, but the bleeding was less than 20cc total, but did pass a walnut sized clot on 12/29 in the ER waiting room. However, finally at 11:45 that night she had an ultrasound where there she was told she had a biseptal uterus, but also a sac containing a yolk. Sent home to chill. New Years Eve was spent in another er, d/t continued bleeding and cramping severe pain. U/s then showed the sac in her uterus with no fetal pole. Last actual period October 3rd, lasting around 4 days. She is quite certain at this point she has lost the baby. But could her cervix be scarred shut, thus necessitating a D&C ? that's my first question. Can she have a D&C simutaneously with removal of the septum in her uterus so she doesn't have to have anesthesia twice:and if so, who does she need to call to do it? We are near Nashville TN. Or should she just continue to chill and take it easy as long as her pain isn't so intense and she has no s/s of infection? If there is any chance the baby could survive this whole ordeal (because her cervix has never opened)? She is just so lost and confused. At one point they told her there was never even a baby there(blighted ovum) Any suggestions would be appreciated.

By the way, my friend is on TennCare (state funded), as her employer doesn't offer insurance. She spent hours today, called 19 different offices (many more OB/GYN's as each office has several docs), and no one could get her scheduled to be seen, much less scheduled for a D&C!!!!!Any suggestions with that?
