I have had a chronic ear infection now for several months.  Antibiotics seem to start to work and then stop.  I have tremendous ear ache (on right side), behind and below the ear hurts, tender, slightly swollen. Low grade persistant fever, headaches on right side, slightly blurry vision in right eye, tight jaw on right side, extreme fatigue, dizzy and horrible sweating especially during activity and at night.   Help!  I can't work, can't sleep...it's awful.

Beth, it sounds like you need an evaluation by an ear specialist (otologist), with possible imaging to rule out a cholesteatoma of the middle ear or mastoid.  This kind of chronic infection demands an evaluation to exclude a surgically treatable cause.
