I did not think the stereotactic biopsy was painful, mostly just uncomfortable. The initial needle used to administer the local anesthesia was like a small stinging sensation; after that I did not have any pain from the procedure. I had two areas biopsied, so it took a little over 40 minutes. Lying on the table without being able to move was probably the worst part for me.
Your breast will be sore for a several days, so wear a good support bra, even to sleep in if you are uncomfortable. Ice packs help a lot for the first day or so.

I understand your concern. The BIRADS is a standardized reporting of mammograms to guide radiologists in the way they interpret the results. A BIRADS of 0 means that the mammogram cannot be adequately interpreted and needs additional tests (in your case it will be a stereotactic biopsy).
Yes, the biopsy may hurt but not more than having an IV access done. I agree that a biopsy is in order considering your family history. Ask your doctor regarding local anesthetics so that this procedure can be as painless as possible.
