People nodding off usually means they are overmedicating. He could try the switch over to Suboxone and then taper down from there. Is he in any kind of support system?
my husband lost a great job doing this.....he tried suboxone bit nodded out on that as well
he is a pharmacutical engineer, not the type of job that you can go nod for a couple of hours
and it would surprise you how many people in my profession do this

One of the several alternatives would be to find a physician that could prescribe for him. This may not be easy since MD's may not maintain an addict. They may prescribe methadone for pain .... if he has any type of pain issues this may work.
Sleeping through a lunch period may indicate that he is doing enough to catch a buzz. And it may indicate poor sleep patterns at night. Benadryl is a good otc sleep aid. Trazadone is a good RX.
The ball is entirely in his court. He will only be clean when he wishes to be clean.
