I would also consider taking her to a neurologist, as well as the psychiatrist. There are times when children exhibit behaviors like you just described, and there is a neurological problem going on. It is the twitching while she is sleeping that she is going through, and that she isn't responding to therapy. There are times when a neurological origin is missed, because it looks so much like psychiatric issues.
It would be worthwhile to discuss these concerns
with the psychiatrist and see what their diagnosis
and suggested follow up work is. Some conditions can
be helped by medication. Many concerns are responsive
to cognitive behavioral therapy alone or in addition to
medication. The psychiatrist should be able to suggest
some ideas. Also family therapy can be of help.
