Could my 3 yr old be bi polar or adhd?

I have a 3 yr old daughter who is above and beyond smart. At 18 months she beagan banging her head on the floor, walls and has even on a cinder block. she can jump in place for about 10 to 15 minutes straight. She says me things like i hate you im going to pack my stuff and go far away. She screams alot for no apparent reasons and when i try to redirect her she hits me or runs away. During bed time she always has excuses as to why she dont want to go to bed like im hungry, i need to use the bathroom. She doesnt nap at all and refuses to when i put her down for one. I cant ever take her out in public like grocery shopping or even to eat because she starts yelling and screaming  and runs away from me. She talks very fast and can go on and on for about 30 min. she now sleep walks and when she is asleep she twitches alot. My partner and i have tried many things and we are at our wits end. she os scheduled to see a physcatrist. She does therapy once a week and it seems like no changes are happening.

I would also consider taking her to a neurologist, as well as the psychiatrist.   There are times when children exhibit behaviors like you just described, and there is a neurological problem going on. It is the twitching while she is sleeping that she is going through, and that she isn't responding to therapy. There are times when a neurological origin is missed, because it looks so much like psychiatric issues.

  It would be worthwhile to discuss these concerns
with the psychiatrist and see what their diagnosis
and suggested follow up work is. Some conditions can
be helped by medication. Many concerns are responsive
to cognitive behavioral therapy alone or in addition to
medication. The psychiatrist should be able to suggest
some ideas. Also family therapy can be of help.
Some posters in the child behavior forum may have
some more ideas as regards parenting.
