VERY possible.
I got Mirena placed January 2011.
IT is now Feb 2013 and I'm holding my 3 month old baby boy, with Mirena still in me. Yes, the same Mirena that was placed 2011.   However, I have a consult with my Dr tomorrow actually, about my upcoming 2nd surgery to attempt to remove the IUD.

My cousin had two ectopic pregnancies while on it, the 2nd one led to her ovary bursting and needing to be surgically removed.

Yes, I got pregnant in my 4th year with the Mirena and subsequently miscarried. If you think you are take a test and call your doctor.
