Yes, it can cause infertility. You have to test. It could be that the pituitary is there, intact and whole but hidden behind the CSF fluid (CSF fluid is opaque in MRIs) or is is squished.
Causes can vary - you can be born with it, you can have "empty sella syndrome, have no pituitary, have high CSF pressure, have a pit tumor...
Risks depend on the hormones that are impacted... some are troublesome, some are deadly. Some impact the quality of life like prolactin, but others will mess up the body like cortisol, growth hormone and thyroid and can be deadly in too much or too little.
