A nipple discharge is significant and needs to be investigated further if it -
- is persistent,
- comes out without squeezing the breasts or any stimulation,
- is bloody or foul smelling or altered,
- is associated with a lump, nipple or skin changes etc.
Because of the significant history of tenderness, it is important that you schedule an appointment with your doctor and see if the discharge needs to be sent for lab analysis to rule out infection and also if any investigations are required.
Has a mammogram been done? Have you noticed any abnormal features on breast self examination like a breast lump, skin changes, etc?

If I were you I would definitely get that checked out. It could be a hormonal reaction to your surgery but if it is frequent, I would have a doctor look at it! God Bless!
