You may also want to have your pancreas checked. That sort of pain along with itch could be an issue with pancreas.
Of course, there are plenty of other things that can cause severe itch. Allergies and/ or medications. But, a peculiar rash along with severe itch is, indeed, a rare symptom of Hodgkin's Disease. My mother's Hodgkin's Lymphoma initially manifested that way... along with drenching night sweats, swollen lymph glands, sudden/ unexplained weight loss and constant fatigue. However, the crushing pain that you described doesn't sound like a symptom of Hodgkin's Disease.
Anyway, the only conclusive way to know what you're dealing with is to have a thorough medical exam and diagnostic tests. Since you don't have medical insurance, have you considered going to a local health center/ clinic? Ordinarily, patients are seen by nurse practitioners... who do a great job in patient care... and the cost is almost always considerably lower than seeing a doctor.
