Savas is right, its hard to say what is going on without more information. Knowing more of his history can be helpful. For example, when was the last time the veterinarian saw him, when was the last time bloodwork was done, and what does he normally eat?
As cats age, just like us, some systems don't work as well anymore and he could be suffering from dry skin or allergies. I wouldn't rule out fleas unless you have actually passed a flea comb through him. Do you have other pets? Dogs that go outside? If so, they could have fleas and they could have started an infestation in your house.

the cat is 14 yrs old he does not have ear mites i check his ears, flea issue hes inside cat. and never go out unless to the vet. i think he has dry skin.

We'd need more information as there's a number of things that could be. It could be a nervous disorder, a flea issue, ear mites, etc...
A bit more information could help point us in the right direction.
