I would add two things;
1. Talk with an attorney BEFORE you give them that Dr's statement. You need to know your legal rights.
2. Not sure what Dr. diagnosed "Parkinson's", but unless you had a solid work up indicating that don't believe it. People with Chiari get told that they have all kinds of terrible things when they DONT. Your Chiari is the most logical explanation and you have proof of that.

Hi Mike, do not forget, there is the ADA Americans with Disabilities Act which means as long as u r willing and able to work they need to offer u a position as long as it dose not cause a hardship for the employer....they have to look for something u can do....and prove there is nothing they can offer,,,

I was very honest with my neurosurgeon about the issues I am still having. I told him about my neck pain, left sided weakness ,dizziness ,fatigue and back pain among other things. He just blew me off. H e said he thinks it's Parkinson's disease. I went to 3 neurologists and 2 of them said I don't have Parkinson's. It's like after he did the surgery he doesn't want to deal with me. He thinks it's Parkinson's because I have tremors on my left side. But I can't get a dr to believe me even when I am shaking when I am sitting right in front of them.
If your neuro says you are fine then you shouldn't have a problem getting a note. Most physicians aren't going to put you at risk for losing your job unless there is a very clear risk to your safety to continue what you are doing. Be very open, honest and up front with your Neuro
