I spoke with my neurologist finally and got answers to my questions. the hypertrophy appearance means "enlarged pituitary gland" could possible be a tumor which is no big deal because they are non cancerous and can be easily removed and only stay over night in hospital.

As for physiologic hyperthrophy appearance - I get a variety of answers when I search this, ranging from scars to the heart. So I really don't know what to tell you. But it sounds like you have a lot going on. Your chiari is growing, and you exhibit symptoms - please contact a chiari specialist. Any nerve damage that is done is irreversible, please don't wait too long to seek appropriate medical help.
