Cerebrospinal Fluid (CSF) Leaks (Intracranial hypotension)
Cerebrospinal fluid leak can leed to inadequate production or too rapid absorption.
Symptoms commonly include headaches, which are more severe in the upright position and are alleviated by supine or head-lowered below chest positioning. Horizontal diplopia, change in hearing, tinnitus, blurring of vision, facial numbness, nausea, and upper limb radicular symptoms (tingling) may occur. These symptoms are non-specific as they are commonly encountered in migraine and post-traumatic headache.
CSF leaks are typically at the level of the spine, particularly the thoracic spine and cervicothoracic junction.
Because of the relationship between CSF and CM/SM a CSF leak is some thing to be considered in a patent work up, and some thing we patents need to be aware of
Syringomyelia (SM) is a disorder in which a cyst forms within the spinal cord. This cyst, called a syrinx, expands and elongates over time, destroying the centre of the spinal cord. Since the spinal cord connects the brain to nerves in the extremities, this damage results in pain, weakness, and stiffness in the back, shoulders, arms, or legs.
Other symptoms may include headaches and a loss of the ability to feel extremes of hot or cold, especially in the hands. Each patient experiences a different combination of symptoms.
Signs of the disorder tend to develop slowly, although sudden onset may occur with coughing or straining.
Neuropathic pain
is a complex, chronic pain state that usually is accompanied by tissue injury. With Neuropathic pain, the nerve fibres themselves may be damaged, dysfunctional or injured. These damaged nerve fibres send incorrect signals to other pain centres. The impact of nerve fibres injury includes a change in nerve function both at the site of injury and areas around the injury. Possible risk during Chari or any surgery
Neurosurgeon:
A physician trained in surgery of the nervous system and who specializes in surgery on the brain and other parts of the nervous system. Sometimes called a "brain surgeon." Also known as a mere mortal being with a severe God complex. These individuals typically have little or no personality or compassion and show little human emotion. This particular breed of physicians have the innate ability to appear attentive while a patient is describing their symptoms without hearing a single word spoken. Amongst their patients, they are commonly refered to as egotisticle royal pains in the butt with personalities likened to doggy dodo.

Drop Attack -
Neurology An episodic and precipitous loss of motor function, where the victim is either standing or walking, and abruptly plummets, fully conscious to the floor, as the legs give way;
idiopathic DAs are most common in older woman, and attributed to age-related defects in reflexes;
DAs may also occur in vertibrobasilar ischemia, acute labyrinthine vertigo, cataplexy, 'plateau waves'; DAs with loss of consciousness occur in syncope and seizures
a form of transient ischemic attack in which a brief interruption of cerebral blood flow causes a person to fall to the floor without losing consciousness. The fall may be caused by a disrupted sense of balance or decreased leg muscle tone. Weakness of the leg muscles or a hip or knee joint dysfunction may be a contributing factor.
Drop Attacks is some thing that most people with ACM are familiar with.

Ok so drop attacks are a group of closely related conditions that come under the one umbrella.
Atonic seizures are a minor type of seizure. They consist of a brief lapse in muscle tone that are caused by temporary alterations in brain function. Atonic seizures can occur standing, walking or sitting, and are often noticeable by a head drop (the neck muscles releasing) and damage sometimes results from hitting the face or head. For the actual seizure, as with common epileptic occurrences, no first aid is needed, except in the instances where falling injuries have occurred. In some cases a person may become temporarily paralyzed in part of their body. This usually doesn't last longer than 3 minutes
Syncope
is the medical term for fainting, a sudden, usually temporary, loss of consciousness generally caused by insufficient oxygen in the brain either through cerebral hypoxia or through hypotension (low blood pressure)
In Chiari Syncope can be brought on by coughing or sneezing
It is not yet understood why people with Chiari suffer Syncope but the general consciences is that it is due to dysfunction of autonomic control of heart function resulting in sudden drop in blood pressure.
Dizziness and vertigo are for the most part vague and non-specific symptoms and hard to define and distinguish between.
vertigo refers to an abnormal sensation in relation to space and position. Vertigo is a specific type of dizziness that is defined as a spinning or rotatory sensation. People with vertigo report that things are rotating around them or that they are rotating around things.
Vertigo could be either from a peripheral (labyrinth and vestibular nerve) or a central disorder (central nervous system). Central vertigo is usually a result of an abnormal processing of the vestibular sensory input by the central nervous system
Chiari associated symptoms of vertigo are a result of the compression of the cerebellum and brainstem structures and stretching of cranial nerves.
Nystagmus
Nystagmus is an involuntary rhythmic shaking or wobbling of the eyes witch usually involves both eyes and is often exaggerated by looking in a particular direction. It is commonly seen in patents with vertigo/dizziness.
There are many different types of Nystagmus with downbeat the most common found in ACM type 1
VNG is a test for Nystagmus
Videonystagmography (VNG) is a test of your balance system. The tests is necessary to diagnose problems that have been determined by a history of vertigo and or dizziness with associated Nystagmus
The exam may include detailed hearing and balance tests. The object of this evaluation is to be certain that there is no serious disease, and to try to pinpoint the exact site of the problem.
