Dr Kennedy what can we do?

Dr kennedy,
My grandson who is eleven has been displaying bad behaviour at school for the last few months.  At home he is fine and no problem.  My daughter has worked with the school and headmaster and yet it still continues.  He has now been suspended from school for 5 days again at home.
He has irrational bad behaviour, even running across the desks on his first day at the school,, or walking out of class and even swearing ( which is not done at home).
He stayed with his brother with my husband and myself during the summer holidays and was no trouble.  We had lots of fun, went to the sea etc. He is afraid of a lot of things that his brother age 13 does.
We also found two pairs of messed shorts hidden in the bed when he went home.
Can you give us any idea as how to approach this?  My daughter has taken him for counselling, he's had a blood test etc
