urinary and bladder incontinence in 7 year old

My daughter has problems with daytime and nighttime wetting, and has recently begun to have problems with BM incontinence.  She was potty trained at the age of 2, but for less than 6 months.  Following divorce she began to have problems with nighttime wetting and occasional daytime wetting.  She does not have full accidents during the day..just dribbles a lot.  To make it even harder to diagnose..she is VERY small for her age.  She was born 6 weeks early and only weighed 4 lbs at birth.  She is almost 7 but is smaller than many 5 year olds.  She has a very hard time with BM's.  When she is able to go..it is extremely large and dry.  A grown man would have a hard time with what she is trying to push out.  Now she has begun to have leakage during the day with no warning.  She says that she cant tell that she needs to go until she is already going.  This is the same response that I get with the wetting.  Doctors have not been very helpful.  When she was 3 I was told not to worry about the wetting until the age of 6.  When she turned 6 I was told not to worry until the age of 7.  Now she turns 7 next month and the problem has gotten worse and includes BM's too!!  This has also been a problem due to odor ALL THE TIME.  Her urine is so pungent that you can smell it in another room after she goes.  Has anyone experienced anything like this?  I need any help that I can get!

Thank you for your advice.  She does usually eat well, she eats a lot of cereal and fruit but not as many veggies as she should.  We have been divorced since she was 2 1/2..and up until 2 weeks ago she was with her father every weekend.  We moved 2 weeks ago to NC from MO.  Her father is still in Missouri.  She seems to be dealing with that very well.  He is a severe alcoholic and it is best that she not be with him until he takes care of this.  After reading everyone's posts here..I have decided to tackle the BM problem first.  I believe that this may be the root cause of the wetting.  I just gave her a laxative to help her go..maybe we can get her regular that way.  I need to find a doctor out here to have her rechecked for bladder or urinary tract infections.  She has come up  negative in the past..but with the odor I have to wonder about that.  Thanks again for your help!  Sorry to be so long-winded!

It would be a good idea to have her checked out for any bladder infections , it also occurs to me that her diet may be causing some problems is she eating enough roughage , cereals,Yoghourts, Fruit, Veg may be she doesnt eat enough bulk, so work on getting her eating patterns good,this will help her 'BMs' ,she could be upset from Divorce if she has been witness to a lot of problems between Mom and Dad. Plenty of reasurrance will help, love ,seeing her Dad regularly, and dont forget to have FUN that is very important in any childs life ..
