Yes.  But in your case, it looks like you had a long follicular phase, rather than a short one, if your period began November 28.

This is what the conception calculator says for a due date of September 12.

     Probable date of ovulation:  December 20
     Possible dates of conception:  December 16 to 24

     Due date:  September 12  (40 weeks)

No.  When a doctor gets a date by ultrasound and it differs from when the woman's last period was, he goes by the date from the ultrasound, because the ultrasound saw and measured the actual baby.  Women can ovulate early or late in their cycle, but a measurement based on the actual embryo is accurate.

Since you had an ultrasound in your fifth week that indicated December 20 as the conception date, I would say it is your ex.
