Bald spots/Hair itchiness

I have an extremely itchy scalp due to raised redness areas. I have a waxy substance attached to the end of my hair when I pull it out.   Sounds crazy but I end up pulling it out because it is so itchy and sort of painful feeling.  I will get a red itchy area on my scalp and then I know that I am having onset and the hair there gets very easily pulled out as if it is dead with black on the ends.  The itchy can be intense.  It comes on without warning (never after stress... I'm basically a stress-free person).     The doctor has checked and there was no infection or fungas associated with the hair or bulb at the end of the hair. Also, the last dermatologist suggested Alopecia Areata but I don't think I have that as my hair is only pulled out to do the issue with my scalp and once all the bad hairs in that area are out, the new hair grows in quickly and healthy as if nothing happened. It is just a daily thing I deal with trying to manage on the new regrowth etc. as I have had this for 15 + year.  I know when a flare up is coming  because I will get a sudden itch and sort of a sore feeling right in that area before it happens.    They often have a black dot on the end of them.   Often two or three hairs appear to be coming out of the same hair shaft and the white waxy bulbs are stuck to one another.  I do not want to pull my hair but do at times developing bald spots because it allows for the the itchy/ pain to go away when the hair is out in that area. Heals up and re-occurs in another spot on my head. I noticed one other person on this forum has had this and then a few others but no one seems to have a clue on what it is!

Sounds very much like what I have been dealing with since I was a child--a bad case of sebhorretic dermatitis. You pull the hairs out because of the intense itching and end up with these bald spots/hair regrowth areas.

I can only recommend washing your hair every day and keeping your scalp clean. My dermatologist prescribed a steroid solution for me to use on the itchy spots when they are unbearable and it's made a big difference.

Accutane blasted the whole problem away, but unfortunately it came back a year later.

Hello,
From your symptoms fungal infection of the scalp should be ruled out. Get an examination and get a KOH examination of the skin scrapings done. If fungal infection is confirmed then you can use topical antifungal shampoos after consulting a dermatologist.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. Possibly 'Black piedra' .Do get a mycological workup. If  identified treatment is simple.
