Rash on Penis, Scrotum and anus. What is it?

I have had this rash for 3 months now. They are random and scattered, one on the shaft of me penis, randomly scattered over my scrotum all of different sizes, and finally the rash follows from my scrotum through to my anus and on the inside of my bum cheeks. I have treated with anti fungal creams, yeast infection creams and tablets. Natural remedies, Coconut oil, tea tree oil and moisturizers. If anything it has spread and now also seems the rash has spread to the front hairline of my scalp. The Rash is a bright red color with defined edges slightly raised, when dry there are fine scales with a dandruff like appearance. It is sore to the touch and if left untreated is very itchy.  I don't have psoriasis anywhere on my body, and no history of it in my family. I have been treating athletes foot. I am a very clean person with good personal hygiene. My sexual partner has no symptoms of any sort on her body. I have changed job recently where i spend a great deal of time sat down, that is the only change in my life. Any ideas what this may be, thank you.

Thank you very much for the reply. After numerous visits to my local GP and a number of anti fungal prescriptions, the rash turned to a brownish coloring and thickened the skin. In returning to my GP I said the only thing that I could think of was Erythrasma to which I asked for Ethromycin Gel however my GP refused and prescribed Frucidic Adic. For the one week I had the cream my skin seemed to return to near normal. However within a week after finishing the tube, the rash had returned. Surely a week is not long enough to clear this condition, as  researching I have found a minimum of two weeks is the norm. A week to fight the infection, and a further week to remove it completely.
I would really like to at least try Ethromycin gel as a course of action, as I feel I have tried all other possibilities and from researching, is a preferred method of treatment for Erythrasma.

I thank you again for your help.

MEDICAL PROFESSIONAL
Hello,
This rash can be due to intertrigo or chafing. An intertrigo is an inflammation (rash) of the body folds. It most commonly occurs due to bacterial, viral or fungal infection. An intertrigo usually appears red and raw-looking, and may also itch, ooze, and be sore.

You should keep the area clean and apply some topical antiseptics. Also wear loose-fitting cotton clothing and use an antibacterial soap or mild soap like dove. Avoid any kind of cosmetics. Topical zinc oxide cream like Sudocrem, Desitin are very helpful(apply with a paper towel and not directly with hands).Application of mild steroid cream also helps. For the itching take some over the counter antihistaminic like benadryl or Claritin. In case the symptoms persist then antifungals may be needed. Please consult a dermatologist in that case.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.
