Open skin wounds on penile shaft

I have had a subincision of my penis for over two or three years now and at the end of it have had a chronic condition of the surrounding skin at the base of the modification  opening, like "slits" or "mouth" shape is the best way I can describe them, on both sides.  They would close in about a couple of weeks time but any kind of extreme pulling or pressure of the skin they would reopen(i.e. masterbating without lube or use of a tight **** ring). The opening and closing of the skin has happened multiple times and it has finally gotten to the point where the wounds on both sides are open and red(but not bleeding or sore, problem is they are also no longer healing.  The last time they opened I decided to try and debride the layer of skin and see if new skin would grow but apparently I have just made matters worse. It seems that they have opened and close so often at this point that my body seems to not want to repair them any longer. I would like to get some opinions on what I need do to get the wounds to close properly.
