i only take the lantus once in the morning never before meals my concern is how much to raise it every two days because the blood sugar leval is coming down finally at 20 units !
No, doses should be raised gradually to avoid risking hypoglycemia. The Lantus is meant to provide background insulin. It does not cover the carbs you eat if you are having spikes 2 hours after meals. You need short-acting insulin for that.

Are you blood sugars high specifically 2 hours after meals or all the time? If they largely spike after meals it sounds like you may need to be on a basal/bolus regimen which includes short acting insulin before meals for the carbs you eat. I absolutely would insist on a referral to an endo if you don't feel your regular doctor has the knowledge to get you stable (they often don't)
