How can I improve my diet accordingly?..

I will try to make this as short as possible, for the past 3 years I’ve been suffering GERD until I had the TIF incision less surgery done in late December 2010. Next 2 months have been heaven but I started noticing new symptoms arising soon after, such as:-
-Difficulty in fall asleep at night which I assume is due to the air being swallowed.
- Frequent Diarrhea and gaseousness and weirdly, it only occurs after lunch time, I have to include that I consume (lentils and Orange juice during and shortly after lunch time)
-Sour taste back of my mouth/throat.
-Fatigue, I’m always exhausted even though I sleep the required 8/9hours a day.
-difficulty burping even when I feel food is stuck in my throat I feel burping just brings it even more up. *I can burp even after the TIF procedure but sometimes I assume it’s impossible due to the food that’s stuck or regurgitated*
-Shortly after consuming water or any other beverage I feel it splashing back up high into my throat.
-Occasional nausea include excess salivation and relieved sometimes by spit up.
A typical days diet: - Morning, cereal with whole milk followed with 2x200ml Galaxy Chocolate milkshakes. Afternoon: - Grilled fish with rice and lentils followed up with orange juice. Mid-noon snack:- 3 Fried chicken fillets followed with more Orange juice. Dinner: - Stir fried chicken with vegetables and soya sauce and cream of mushroom soup followed with a cocktail juice drink.
I typically will nibble on some chocolate bars in between (1 or 2 MAX) and lots of water.
I have been also diagnosed with (Irritable Bowel syndrome) after theoretically describing to my Doc about symptoms such as the Diarrhea and flat tight abdomen in mornings accompanied by nausea.
All I need is how do I place the perfect diet, would I appreciate some suggestions!
Most importantly I’m a 19 year old adult, other features= 5foot8 and 63kgs so I’m dying to put on more weight...Can I use weight gainers for example!

  Hi...I do not know y u drink so much orange juice,  except bcuz u like OJ, BUT it is acidic...not good for one with GERD issues.

U r also eating fried foods...20mg of nexium when u r still consuming foods that u really should be avoiding....chocolate is on the list to avoid,,, cream of nething is too much as well....

Forgot to include, I still 20mg nexium which I pressume has lost its effectiveness..
