I had a ct scan, which showed extrahepatic biliary dilation.

I had my gallbladder removed in 1989.  For several years my right upper abdomen has pain at all times and sometimes it is really bad. My liver is enlarged and very sensitive to the touch. Please help me understand what this means ?

The basics of this can be seen in your blood work. The bile flow impedance will show anything strange via the indirect and direct bilirubin levels - and the internal pressure and stress your biliary system endures will be reflected in your ALP levels (alkaline phosphatase). If the bile cannot exit your liver properly, then your liver enzyme levels (ALT, AST, GGT) will increase ... back flow of your bile is basically a poison and will kill your liver cells.

Make sure your doctors do a consistent monitoring of the liver panel and CMP, with the blood work draws they do, at least every 6 months or so. Anything crazy going on will be detected.

Hopefully your body will have fully recovered, and you are on your way to a perfectly normal, and long healthy life.
