If you are out a week from the onset you probably don't need to do anything as it will gradually resolve on it's own. The pain should reduce significantly but the lump may take 6-8 weeks to go down. If we see patients within a week of onset and with significant pain we will make a small incision and evacuate the clot from the hemorrhoid. This usually gives immediate relief.

You would appear to have a thrombosed external hemorrhoid. This could be drained by making a small incision and removing the clot or it will gradually resolve by itself.
