Need help for grandmother/custodial parent of 6 yr. old ADHD boy!!!

Why does my 6 yr old grandson throw himself down on the floor and almost continuously make noises while watching TV? Is this Tourettes Syndrome? He was diadnosed with ADHD a year ago. Why does he suck on his knuckles on his fingers? I also have a very hard time getting him to sleep.
I have had custody of him since he was 1.
His mother and father were both Drug addicts. Meth, RX pain meds, Crack Cocaine. His mother was on drugs during the entire pregnancy.
My grandson also has a 7 & 8 year old brothers who lives in individual households.
I get so tired of saying quit, stop, don't and let's have a talk, which absolutely does NO GOOD!!

It sounds to me like he might have some form of Autism. In which case, TV and other things can cause negative reactions. It's also not uncommon for kids with Autism to have compulsive things, like knuckle sucking. Guiding and directing them is often useless and a waste of time. He does need to be evaluated and there is help for him, and you. Kids with Autism need a structured life and when things fall out of order, they often become upset and act out. The sooner you find out what is going on with him for sure, and get some help, the better off the both of you will be. Good luck and God Bless you both!

At the very least, please unplug the television.  The flickering images on screens have been shown to affect the wiring in young children's brains, not for the better.  

"Screen time" has been shown in scientific studies to make behavior of kids with ADHD worse.  He probably does this while watching TV because his poor brain is overstimulated.  You might find it challenging to keep him busy without TV, but for the sake of his ability to think, make decisions, and function in life, please don't allow him to watch TV, movies, or play video games for the next several years.
