Condom broke - Insertive partner- PEP Started - HIV Risk?

Thank you all first off for taking time to read my post. I am backpacking and far away from home and have no one to talk to.

I recently met a guy and we were having sex and the condom broke. I was the top, insertive partner. There was no lube, it was not rough. It lasted maybe 3 minutes with moderate thrusts until I looked down to see the condom broken and my entire penis exposed. There was no blood that I could see. I am American and this man is Malaysian. He lives away from the major towns but said he comes down to KL to have sex when he needs it. Therefore, it seemed like he came down here quite often for sex and has many sexual partners. He is a "complete bottom" he said. He told me of stories of him with paid sex in the past which also scares me. He claims he is clean but it wasn't a sure thing by any means.

Anyways, I have only had anal sex (insertive) a handful of time so I am rather new to this. It scared me to death. I went to the clinic and they started me on a round of PEP.

I am on Tenvir-EM and Stocrin which are the brand names. The active ingredients of you will are tenofovir, emtricitabine and efavirenz. I am on day two of the Meds. The first night the side effects of dizziness were extremely severe. It was hard to walk. I found myself having a panic attack until I finally just went to sleep. When I woke up I was rather fine and did not feel dizzy. The dizziness went away by today, day 5, but my lower back is in SEVERE pain.

My question:
1) Do these side effects ware off as the course goes on? Will it get easier? or will it get worse as I continue taking more and more medicine is in my bloodstream.
2) Is there any long term side effects for taking these specific drugs if I wasn't exposed to HIV during the incident. (Because I am not 100% sure the man was or was not positive).
3) Why is my lower back in severe pain?

I am staying in hostels and all I have is my backpack so this is har to go through a PEP course when traveling but I will do ANYTHING for my health safety. I am extremely lost and scared at the moment and any feedback is appreciated.

Unless you have kidney or liver issues that shouldn't be a problem. Like I said if at all possible and realistic change out the Stocrin. If you are close to completing the 28 days it might not be worth the fuss.

The pain comes in waves at no particular time. Sometimes in the morning and others in the afternoon.

Yep, taking then right before bedtime. Thank you for your reply. The back pain is the only thing that worries me. It feels like an organ is receiving damage. Never felt this before.

Are you taking your meds a prescribed? Stocrin needs taken on an empty stomach and taken at bed time so you sleep through the side effects. Your other meds won't be affected by food but it is easier to take them at night so you can keep track of them. I was on meds close to the same thing for 7 years and put up with the side effects I was uninformed and I didn't know that you were not suppose to have those side effects. I  went through muscle pain, nausea, and dizziness especially if I had to get up and go to the bathroom 20 minutes after taking them. After my ID doctor and I had a discussion mine was changed. You shouldn't be going through that much misery taking those meds. If you can't tolerate it, I would see about a different combination of meds or at most change out the Stocrin. The Stocrin is the culprit in your side effects. Good luck to you.
