I still strongly recommend you see your doctor regarding what you feel may be a blockage in your suprasternal notch with difficulty swallowing.

You said the condom slipped, but did NOT come off or break. As long as the head of your penis was covered, you had no risk.
Fingering is NOT a risk for HIV.
