PEP or not- 36 hrs in

About 36 hours ago I was with an escort who had been giving me unprotected oral as I lay on my back with my eyes closed.  She stopped, fumbled around and I felt her putting the condom on with her mouth.   She hops on and we start going and I'd say 20 seconds pass when I realize that she didn't put a condom on me.   I stopped and asked for one but she didn't have any.  That freaked me out and I ended up leaving and with the lights not dim as I I got a better look at her and she seemed like more than a recreational drug user.  

I went to a clinic today and the Dr.  didn't recommend PEP relative to my risk but gave me a prescription for it as it's ultimately my decision.   He did give me 2 gram one time dose of antiobiotics in case of gonorrhea or clamydia  . This woman is having sex without condoms maybe routinely and maybe a drug user so i guess I have this 72 hour window to decide and having trouble weighing the risk vs. the PEP course.which has it's own issues I guess.

Usually 20 seconds of vaginal intercourse wouldn't warrant PEP, however, if you feel as though she was a careless person and a drug user, PEP might not be a bad option.

If you decide to go through with it, then take the DUO at 4 weeks post last dose of PEP.

Personally, I would follow up with an antibody test at 12 weeks post last dose of PEP for a conclusive result.

thank you for the response.  I've been trying to weigh the length of time of insertion into the level of risk.  20 seconds vs. say 5-10 minutes.   I never even got fully erect inside her which I don't if that factors in or not.

Reading some dr. responses for questions about condom breaks and PEP being way overly aggressive led me to think it's a low risk and I can just wait and test later but since I am in that window where I could use PEP it's tricky.

You did have a risk. PEP is entirely up to you. The sooner you start the more effective it is.

However if you start PEP it will delay the time of getting accurate/definitive results.

If you choose not to start PEP. An HIV Duo test done 4 weeks or more after exposure will give you conclusive results. You need to weigh the options and decide. PEP must be started quickly if you decide. Waiting longer makes it less effective.
