Condom breakage

mmm my story is that i had sex with a girl who do not seem to care about safe sex and these stuff
we did protected intercourse oral and vaginal with the same condom i don't know about the expiry or the quality
but it was latex and it broke on the shaft side and the head was covered
the second time was two weeks ago the same thing covered both acts with the same condom however after about 5
min of oral i had a penetration for about 5 seconds and remembered to change the condom for each act so i pulled out
and tested the condom half filled with water and seemed to be not leaking. the problem i started to feel a pin in groin area with low grade fever
what is your assessment about the two incidents?
