24 year marriage ended and dating

Hello.
53 year old. I ended a 24 year old marriage. Recently started dating. My husband and I never had use for condoms and there really was no need for them.  I am seeing someone new and we spoke about protection.  I have a few questions regarding latex condoms and protection:

1. When are condoms not effective in protecting against HIV?
2. Do I need to test for HIV even though we use protection?

What I am trying to say is this...besides a condom "breaking" or "slipping off during" sex, what else am I missing that would make condom use "unsafe"?

I am aware of that. Would the remote chance that he a fraction of the condom on withdrawal compromise my safety?

Had the condom failed you wouldn't be saying unsure you would know without a doubt.

we had intercourse. I am unsure if the condom we had used was compromised in any way.  

What would make the circumstances "not safe" with a condom?
